What the inspector wrote

Based on LPA investigation and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Pursuant to Title 22 of the California Code of Regulations, the following Type B deficiency was cited (refer to LIC 9099-D). Exit interview was conducted with Director Mearia Tarver via tele-inspection, during which appeal rights were explained. This report along with a copy of the appeal rights and Notice of Site Visit (LIC9213) will be sent to the Director via email with a read receipt or confirmation of receipt of email, which will act as the Director's signature. The LIC 9213 Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty.
