What the inspector wrote

On 04/29/26 at 12:30 PM, Licensing Program Analyst (LPA) Veronica Martinez conducted a Case Management–Incident inspection at the Child Care Center (CCC) to follow up on an Unusual Incident Report (UIR) received by the Department on 04/14/26. The reported incident involved two children in care who were outside during playtime. Child 1 (C1) hit Child 2 (C2) with a wooden block causing an injury to C2’s forehead. LPA met with Site Lead Lupita Reyes to discuss the purpose of today’s inspection. LPA observed that 19 children and 4 staff were present at the time of the inspection. Site Lead informed Licensing that C2 sustained a cut to the forehead and required medical attention. Site Lead stated there were 3 staff present and within ratio at the time of the incident, and staff were in close proximity to C1 and C2 when the incident occurred. Site Lead stated staff were less than 4 feet away. The site lead reported that the incident occurred at 10:00 AM and both children’s parents were notified right away. Staff cleaned the cut and applied ice and pressure to C2’s forehead. The parents took C2 to Urgent Care, where medical glue was applied to close the wound. LPA and the Site Lead discussed active supervision. The Site Lead reviewed the incident to identify contributing factors and implemented corrective steps to help prevent a recurrence. CONTINUATION OF LIC 809-C LPA investigated this incident and, based on interviews conducted with the Site Lead, it was reported that C1 and C2 were participating in outdoor play at the time of the incident. Site Lead stated staff were supervising children in the area and responded immediately when C2 was injured. Staff followed center procedures by providing first aid, notifying the parents without delay, contacting Licensing within 24 hours of the incident, and submitting a follow-up Unusual Incident/Injury Report (LIC 624) within the required 7-day time frame. Information provided during the interview was consistent with the details documented in the Unusual Incident Report. Following the incident C2 returned to the CCC two days later and is currently still enrolled. An exit interview and review of the report were conducted with Site Lead Lupita Reyes. A Notice of Site Visit was provided and must remain posted for the next 30 days.
