Inspector’s narrative

What the inspector wrote

with not opening in the evenings and that they had submitted a work order. Director stated that they updated the work order on 9/15/2023, when the door would no longer able to be opened from the inside. Staff interviews corroborated that the gate had only been malfunctioning completely for a few days and that children are inside during drop offs and pick ups. Interview with Staff (S1) also revealed that the Director will also sit on a bench outside the gate to open the gate for parents. LPA reviewed the work order submitted by the Director and confirmed that it was submitted and updated as stated previously. LPA notes that the work order has been received and and accepted and that it is scheduled for 10/11/23. During the inspection Director called their supervisor to get the work order expedited and informed LPA that the gate is now scheduled to get fixed on 9/26/23. Based on interviews and document review, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Because the Director has a plan in place to fix the gate and the corroboration from staff interviews that the children are inside when the gate is propped open, no deficiencies are being cited. A Techincal Violation is being issued on the attached LIC9102.
