What the inspector wrote

Licensing Program Analyst (LPA) Veronica Diaz conducted an unannounced Case Management to the above mentioned Child Care Center (CCC) to deliver an Accusation/Exclusion Action (CDSS No. 7924135003) in the matter of staff, Daniela Shadi Page. LPA met with Director Amanda Holloway and explained the purpose of the inspection. There were 74 children, and 11 teachers present during the inspection. The Accusation/Exclusion Action was discussed with the Director, Amanda Holloway, which indicates CCLD's intent for respondent, Daniela Shadi Page, to be prohibited for the remainder of respondent’s life, from employment in, presence in, and from contact with clients of, any facility licensed by the Department. LPA confirmed the respondent was never employed at this facility the CCC has no record of the respondent . Director stated there must be an error with the facility number associated to the respondent. LPA advised Director to disassociate respondent, Daniela Shadi Page, from the facility roster immediately. The following documentation was provided and explained: · Accusation CDSS No. 7924135003 No deficiencies were cited during today's inspection. A Notice of Site Visit (LIC 9213) was issued and must remain posted for 30 days or a civil penalty of $100 may apply. Appeal Right (LIC 9058) was provided to the Director.
