What the inspector wrote

On 09/11/25 at 2:57 PM, Licensing Program Analyst (LPA) Fernando Hernandez conducted an unannounced Annual inspection. LPA was advised by the elementary school office staff that the YMCA is located within the multipurpose room. LPA met with site supervisor Aileen Celis and advised them of the purpose of the inspection. Site Supervisor provided LPA a tour of the center inside and out. There were 16 children in care at the time of the inspection under the care of (2) staff. This site is an after-school program. Center is open M-F, from 1:00PM - 6:00PM children. LPA observed required licensing documents pined on the wall at the entrance of the multipurpose room. Last fire drill was conducted on 08/26/25. The multipurpose room has age-appropriate toys/equipment, activities, and tables accessible for children in care. LPA observed school site restrooms available for children to use. LPA observed a container of "Super Cleaner within the children's restroom with (2) children present at the time which was accessible which posed an immediate risk to children in care, a TYPE A deficency was cited based on observation (please see LIC809-D for further details). First Aid kit was observed in the cabinet by the entrance of the multipurpose room. Currently the center uses an App called ePact for sign-in and sign-out. Site supervisor stated the school provides monthly newsletters with snack menu. Site supervisor informed LPA the school provides an afternoon snack. Center uses school site outdoor area for outside play and outside activities. The playground has age-appropriate toys and structures available for children to use. Continued on 809-C PAGE 2 A sampling of 4 children and 4 staff records were reviewed. LPA observed children's files to be complete and current. (2) Staff mandated reporter certification were reviewed and found (1) staff had a current mandated reporter training certificate current however (1) staff did not have a current certification on file. A TYPE B deficiency was cited based on observation (please see LIC809-D for further details). LPA verified SB792 Child Care Adult Immunization and Tuberculosis requirements. Site supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Site supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During today's inspection (1) Type A & (1) Type B deficiencies were cited during today's inspection. (Please see LIC809-D page for further details). CONTINUED ON LIC809-D PAGE 3 LPA Fernando Hernandez informed site supervisor Aileen Celis that this report dated 09/11/2025 document(s) (1) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Fernando Hernandez informed the site supervisor to provide a copy of this licensing report dated 09/11/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights was explained and provided to Licensee. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the site supervisor Aileen Celis.
