On October 3, 2025 at 10:15 AM Licensing Program Analysts (LPAs) Laura Carone and Brian Fung conducted an unannounced Case Management-Incident inspection. LPAs met with Head Teacher, Priyadarshanie Marikar and explained the purpose of the visit. LPAs conducted a tour of the facility inside and outside with Head Teacher. LPAs observed a total of 17 children under the care and supervision of 2 staff. Director/Owner Roshini Wirekoon arrived at a later time. An incident that occurred on July 17, 2025 involving an infant (22 months old) that was moved into the preschool classroom per parent request. LPAs explained that an infant must stay in the infant room unless a written request (waiver) has been approved from the Regional Office. The center staff did not obtain an approved waiver, and therefore the infant was commingled with the preschool children. The incident was not reported to the Regional Office by the center staff. LPAs informed Director/Owner, Roshini Wirekoon that unusual incidents are to be reported via telephone call within 24 hours of occurrence and in writing within 7 days. LPAs provided Director/Owner with a copy of an LIC624 Unusual Incident/Injury Report to complete. A type B citation is being issued. LPAs Laura Carone and Brian Fung informed Director/Owner that this report dated October 3, 2025 documents 1 Type A citation which shall be posted for 30 consecutive days as there are immediate CONTINUED ON LIC809C risks to the health, safety, or personal rights of children in care. Also, LPAs Laura Carone and Brian Fung informed the Director, Roshini Wirekoon to provide a copy of this licensing report dated October 3, 2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal rights given.
