Pre-licensing visit·March 24, 2021

This is a change in ownership for the previous facility number 197420010 effective 03/01/2021. On March 23, 2021 at 10:15 am, Licensing Program Analyst (LPA) conducted a pre-licensing change of ownership inspection. LPA met with owner Bishan Seneviratne and facility Director Lorelle Stevens. LPA advised them the purpose of the inspection. Due to COVID-19 and the Department of Public Health guidelines, a tele-inspection was completed via Facetime. LPA was provided a tour of the facility inside and out. The center hours are Monday – Friday 7:00 am – 6:00 pm. Currently the facility is on a modified schedule due to Covid -19 Monday – Friday 8:00 am – 5:00 pm. There were four (4) children in care at the time of the inspection. This is a combination center with a preschool component. LPA observed required licensing notices posted prominently on the wall in the office. The facility will be using five (5) classrooms for children in care. The classroom had age appropriate toys and furnishings. LPA observed one sink with a changing table within one arm reach readily accessible. LPA observed cribs for infants and mats available for the older infants that do not use cribs. Food and milk will be stored in refrigerator and/or freezer located in the classroom. LPA advised Administrator to ensure the food and milk is appropriately labeled with the child's name and date. LPA advised Administrator to ensure every infant has an updated needs and services plan for care. LPA did not observe any toxins/hazardous items accessible to children in care. That facility has two (2) outdoor recreation yards for the infant component. LPA observed age appropriate structures and play equipment readily accessible to children in the outdoor recreation area. The facility has ample amount of shade. The facilities RAST inspection was completed on 08/26/2020. The Incident Medical Services policy was discussed. Currently the facility does not have any children that require (IMS). Continued on 809-C Facility Measurements: Indoor – 1,761 square footage meets the requirement for 24 children. Outdoor – 4,260 square footage meets the requirement for 24 children. The center meets Title 22 Division 12 requirements. License will be effective today, March 24, 2021 for 24 infant children. A copy of this report was reviewed and provided Director Stevens. Director agreed to receive a copy of report via email and voiced understanding that the read receipt confirmation from email will be in lieu of her signature once she receives the report. LPA requested Director send a signed copy of the report to licensing for records. THE NOTICE OF SITE VISIT WAS POSTED AS REQUIRED BY H&S CODE SEC. 1596.817. THE NOTICE OF SITE VISIT MUST REMAIN POSTED FOR 30 DAYS OR A CIVIL PENALTY OF $100.00 WILL APPLY.
