What Happened
Complaint investigation substantiated three serious violations: children using compost potties in classroom due to ongoing plumbing issues since November 2024, continued access to restroom facilities while undergoing sewage mitigation, and failure to report incidents as required.
Observations
- Children using compost potties in classroom rather than designated restroom due to plumbing problems
- Sewage overflow/water damage in designated restroom area (approximately half inch of water)
- Ongoing plumbing issues since November 2024
- Children present during sewage mitigation services on 12/06/2024
- Facility temporary closure due to plumbing problems
- Plumbing and restoration services provided 10/31/2024, 12/06/2024, and 01/08/2025 while children in attendance
- Incidents not reported in Unusual Incident Reports (UIRs) as required
