What the inspector wrote

On 02/26/2026 at 9:20 AM , Licensing Program Analyst (LPA) Veronica Martinez (Diaz) and LPA Cynthia Alvarez conducted an unannounced Required – 3 Year inspection at the Family Child Care Home (FCCH). LPAs met with the licensee’s mother and assistant and advised them of the purpose of the inspection. The assistant stated the licensee was on her way home from an appointment. The licensee’s mother provided LPAs with a tour of the FCCH, both inside and outside. There were two children in care at the time of the inspection. Licensee Veronica Carillo arrived at the FCCH at 9:40 AM. At 10:30 AM, an additional child arrived at the facility. The FCCH operates Monday through Friday from 7:00 AM to 5:30 PM. LPAs observed required licensing documents posted on the parent board. LPAs reminded the licensee that the parent board must be posted in a location that is visible and accessible to parents/guardians at all times. The licensee stated that fire and earthquake drills are conducted; however, licensee was unable to provide documentation verifying that drills are conducted at least once every six months. The licensee stated the last drill was conducted in March or April 2025. LPAs reminded licensee disaster drills needs to be conducted every six months and documented. A 2A10BC fire extinguisher was observed mounted on the kitchen wall. The licensee stated the purchase receipt was lost. Fire and carbon monoxide detectors were tested during the inspection and were initially not functioning. The licensee replaced the batteries, and the detectors were operational at 10:03 AM . Continued on 809-C Children in care have access to the living room/day-care room, one restroom, and the backyard. LPAs observed off-limits areas that were accessible to children (family room and Kitchen). LPAs inspected these areas and reminded the licensee that off-limit areas must remain inaccessible to children during operating hours. During the inspection, LPAs observed the following hazards accessible to children: medication on the kitchen counter, a knife block on the counter, cleaning agents stored under the kitchen sink. These items pose a safety risk and must be made inaccessible to children in care. The fireplace in the family room was blocked by a bookcase to prevent access by children. Age-appropriate toys were observed indoors and outdoors. The backyard play area is properly enclosed and contains toys, play structures, and activities. During the tour, LPAs observed a gas can in the side yard. LPAs reminded the licensee that hazardous materials and toxic substances must be stored that makes them inaccessible to children in care. LPA reviewed 3 children’s files. LPAs reminded the licensee that updated immunization records for all non-school-age children must be maintained in each child’s file. LPAs also reminded the licensee that infants under 12 months of age are required to have a completed LIC 9227 Individual Infant Sleeping Plan and 15 minute safe sleep logs. The licensee’s CPR/First Aid certification is valid through 04/28/2027; however, the licensee completed the course online. LPAs reminded the licensee that Pediatric and Adult CPR/First Aid training must be completed in person. The licensee was also reminded that a licensed child care provider, administrator, or employee of a licensed child care facility must complete the Mandated Reporter Training (AB 1207) every two years at www.mandatedreporterca.com . The licensee’s Mandated Reporter Training expired on 09/13/2023. Continued on 809-C Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPAs Veronica Martinez (Diaz) and Cynthia Alvarez informed licensee Veronica Carillo that this report dated 02/26/26 document 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care. Continued on 809-C Also, LPAs Veronica Martinez (Diaz) and Cynthia Alvarez informed the licensee to provide a copy of this licensing report dated 02/26/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Veronica Carillo , confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
