Inspector’s narrative

What the inspector wrote

On 11/18/2025, at 9:15 AM, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Annual/Random inspection of the above-mentioned Child Care Center (CCC). LPA met with Mona Golastani, Licensee of the CCC and explained the nature and purpose of the inspection. LPA in the company of the Licensee, toured the interior and exterior of the CCC. The CCC operates from 8:00 AM to 3:00 PM, Monday through Friday. The CCC is a combined center with a preschool and school age program. The preschool program currently utilizes one classroom (Room 1), while the school age program uses Room 4. LPA notes Rooms 2, 3, 5, and 6 are not currently being used. LPA observed seventeen (17) children during the inspection, along with two (2) teachers (cleared and associated) providing care and supervision. LPA observed required licensing forms and documents posted on the wall of the CCC\342\200\231s office. LPA observed age-appropriate toys and furnishings throughout the CCC. The CCC\342\200\231s preschool program provides snacks while children in care bring lunch from home. School age children bring snacks and lunches from home. LPA observed carbon monoxide detectors throughout the CCC. A carbon monoxide detector was tested at 9:20 AM and found to be operable. Children in care have access to available drinking water both inside the classroom and outside in the play area. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. Continued on 809-C For child care center licenses issued after July 1, 2022, the Licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Medications, sharps, and cleaning supplies were observed to be in areas of the CCC that are accessible to staff and inaccessible to children in care. The restrooms in the CCC are in clean and sanitary operating condition. The CCC uses ProCare for Sign In/Sign Out purposes. At 10:30 AM LPA notes that C1 and C4 were not signed in by their authorized parents/guardians. The CCC\342\200\231s outdoor play yard is fully enclosed, and the fence\342\200\231s entry/exit points are secured. The footing in the area is varied with artificial turf and sand to absorb falls. The play yard provides ample shade to afford for the children\342\200\231s comfort. LPA observed age-appropriate toys/play equipment in the play area which are in suitable condition. LPA observed waste receptacles with tight fitting covers in the CCC. Staff and children\342\200\231s records were reviewed. At 11: 00 AM, LPA notes the nine (9) children in the school age program do not have any licensing forms on file. LPA notes the remainder of the children\342\200\231s records are current and complete. Staff records were reviewed and found to be current with Mandated Reporter trainings and staff qualifications. Licensee states there are no firearms or ammunition stored on site. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 159 6.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Today, two Type B deficiencies are being cited under the California Code of Regulation Title 22 (see 809-D). A notice of site visit was given and must remain posted for 30 days.
