What the inspector wrote

This is an amendment to October 8th, 2024 report to reflect outdoor play space measures at 1,894.71sq. and per applicant and director, no bodies of water are present on property. On October 8th, 2024, Licensing Program Analyst (LPA) Rosie Breault conducted an announced prelicensing inspection at the above aforementioned center to ensure that health, safety, and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. LPA met with applicant Mahkameh Mazarei and director Gina Fernandez. There were zero children at the time of the inspection. Applicant applied to provide care and supervision to 24 toddlers aged 18 months \342\200\223 36 months and 30 preschoolers age 2 years to 6 years. Facility will operate Monday \342\200\223 Friday 7:30AM-5:30PM. Applicant, director and LPA toured and measured the on-limits areas and classrooms located at the facility. LPA observed required postings placed in a prominent location, and parents / authorized representatives will use Brightwheel App for the purposes of signing in and out, and LPA observed back up written logs. The facility to utilize 4 classrooms designated as: Toddler Room #1, Toddler Room #3, Preschool Room #2 and Multipurpose Room. There are 4 toilets and 4 sinks available for use and functioning at the time of the inspection. The facility was observed to be clean, safe, sanitary, and LPA advised must be maintained in good repair at all times to ensure the safety and well-being of children, employees, and visitors. The facility offers AM and PM snack for children and menus shall be in writing and shall be posted at least one week in advance in an area accessible for review by the child's authorized representative. LPA observed floors of all rooms have a surface that is safe and clean. Furniture and equipment were observed to be in good condition, free of sharp, loose or pointed parts. Tables and chairs were observed to be meet the needs of the children. At the time of the inspection, all toxins, cleaning supplies, poisons were rendered inaccessible to children, in a separated locked cabinet. No toxins and poisons are located in the classrooms. Carbon monoxide detector present and functioning at the time of the inspection, fire extinguishers were last serviced on 9/24/2024 and first aid kits available. Per director, no firearms or ammunition are present on property. Los Angeles County Fire cleared facility on 10/3/2024 and LPA advised applicant and director facility is comply fire occupancies at all times. The toddler classrooms contains age-appropriate toys and equipment. Classroom houses 1 changing table with vinyl covering within arms reach of a sink. LPA observed cots for sleeping and individual cubbies for personal items. LPA observed a range of activities for children, access to water and comfortable room temperature. LPA advised director that 15-minute sleep checks are required until 24 months which must be logged daily, with condition and position of child recorded. Safe Sleep is to be adhered to at all times. The preschool classrooms contain age-appropriate toys, equipment, tables, chairs, bookshelves, sufficient number of cots for napping, and cubbies for individual items. LPA observed comfortable room temperature, access to filtered water, ample play activities for stimulation. The outdoor area consists of one main yard and side yard with ample shade, water readily accessible, soft padding to absorb falls, permitter fencing, appropriate toy structures and tricycles Drinking water from a noncontaminating fixture or container shall be readily available both indoors and in the outdoor activity area. LPA advised per Health and Safety Code 1597.16(a)(1) Lead Testing and PIN-21-21-CCP Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every 5 years after the date of the first test. Per Written Directive 100700(c)(1) Written Directives for Lead Testing (1) For a license issued on or after July 1, 2022, initial testing results shall be received and posted within 180 days of licensure. Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. The total facility indoor capacity measures 2575.43 square feet which accommodates applicant\342\200\231s total facility requested capacity of 54 children. Total toddler classrooms capacity measure 1079.05 square feet which accommodates applicant\342\200\231s requested capacity for 24 toddlers. Total preschool classrooms capacity measure 1243.74 square feet which accommodate applicant's requested capacity for 30 preschoolers. Applicant to submit outdoor play space waiver.
