What the inspector wrote

On 02/12/26 at 9:30 a.m. , Licensing Program Analyst (LPA) Veronica Martinez (Diaz) conducted a Case Management \342\200\223 Incident inspection at the Child Care Center (CCC) in response to an Unusual Incident Report (UIR) received on 10/08/25 . The UIR alleged that a child in care ( C1 ) reported a male staff member asked C1 to touch themselves in the genital area, which C1 referred to as a \342\200\234winkie,\342\200\235 identified by the parents as meaning a penis. It was further alleged the individual showed C1 images on a cellular phone, including photos of himself with friends and pictures of Godzilla. LPA met with Director Johanna Pimentel and explained the purpose of the inspection. At the time of the visit, 36 children and 9 staff were present. The Director reported that C1\342\200\231s parents notified the center after C1 made the statement to them. The parents were unable to identify when the alleged incident occurred and acknowledged the possibility the statement may not be factual. The LPA and Director discussed active supervision . Continued -809C The LPA conducted an investigation that included staff interviews, record reviews, and observations. Based on the information obtained, there was insufficient evidence to substantiate a lack of supervision. The center complied with licensing requirements by notifying the Department, submitting a UIR, and making a report to Child Protective Services (CPS). LPA advised the Director to contact local law enforcement should an incident of this nature occur in the future. Information obtained during interviews was consistent with the UIR. No deficiencies were cited during today\342\200\231s inspection. An exit interview and report review were conducted with the Director Johanna Pimentel . A Notice of Site Visit was issued and must remain posted for 30 days.
