What the inspector wrote

On 12/29/25, Licensing Program Analysts (LPAs) Veronica Diaz and Cynthia Alvarez conducted a Case Management\342\200\223Incident inspection at the Child Care Center (CCC) to follow up on an Unusual Incident Report (UIR) received by the Department on 12/23/25. The reported incident involved a child in care, C1, who was outside in the play yard running resulting in C1 tripping and falling. C1 hit their head on the edge of a child size picnic table causing about one inch gash on the forehead. LPAs met with director Nicola Adams to discuss the purpose of today\342\200\231s inspection. LPAs observed that 33 children and 4 staff were present at the time of the inspection. Director informed Licensing that C1 sustained a cut to the forehead and required medical attention. The Director stated that Staff were present and within ratio at the time of the incident. The ratio was 22:3, and the age group was 2 and 3 year old children. The Director reported that the incident occurred at 3:30 PM and that the parents were notified. Staff used the call box to request assistance, then used the center\342\200\231s first aid kit to clean the wound and applied ice and pressure to C1\342\200\231s forehead. The parents took C1 to the doctor C1 received 3 stiches. The Director stated that staff were reminded of the importance of active supervision and were instructed to redirect children from running too close to equipment to help prevent injuries. CONT 809-C LPA observed were this incident took place and based on interviews conducted with the Director and staff, it was reported that C1 was participating in outdoor play at the time of the incident. Staff stated they were supervising children in the area and responded immediately when C1 was injured. Staff followed center procedures by providing first aid and notifying the parents without delay. Information provided during interviews was consistent with the details documented in the Unusual Incident Report. Following the incident, C1 continues to be enrolled at the CCC. During today\342\200\231s inspection no deficiencies were issued. An exit interview and review of the report were conducted with Director Nicola Adams. A Notice of Site Visit was provided and must remain posted for the next 30 days.
