What the inspector wrote

On December 29, 2025, at 12:29PM , Licensing Program Analysts (LPAs) Cynthia Alvarez & Veronica Diaz made an unannounced visit for the purpose of conducting a Case Management - Incident inspection. LPAs met with director Nicola Adams to discuss an incident that was self reported to Community Care Licensing Division (CCLD) office by telephone call on December 11,2025. LPAs toured the hallway area where the incident occurred and interviewed the director. During the time of the inspection, there was 33 children in the care and supervision of 4 staff. On December 10 2025, from 12:20 PM until 12:20 PM, 3 children (C1, C2, C3) were left unattended in the hallway due to teacher (S1) having to go after another child (C4) that ran down the hallway. When S1 ran after child, and brought C4 back to the classroom, the door behind them closed automatically and left C1, C2, and C3 unattended in the hallway for approximately 10 seconds. LPAs observed the hallway and the classroom the incident occurred, the distance between where the children were left unattended and where the classroom is located is in close proximately. The parents of all the children involved were notified of the incident and all the children returned to the center the next day. To prevent similar incidents, the facility staff were reminded of active supervision strategies and continue to use the \342\200\234face-to-name\342\200\235 procedure which entails a check-in system anytime the children transition from one area to the next. LPAs advised director of the importance of active supervision for all staff. Given the children were left unattended for approximately 10 seconds, and staff was in proximity of the classroom. No Citation will be given during today\342\200\231s inspection. A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Appeal Rights were provided to Site Supervisor Exit interview conducted and report was reviewed with Director Nicola Adams.
