Inspector’s narrative

What the inspector wrote

On 02/17/2026, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Case Management inspection of the above-mentioned Child Care Center (CCC) to follow up on an Unusual Incident Report (UIR) received by the Department on 02/04/2026. Specifically, a child in care (C1) sustained a cut to their forehead. The incident occurred when C1 fell while walking down the steps of a play structure. LPA met with CCC Director Nicola Adams and explained the nature of the inspection. LPA notes sixty-five (65) children are on site along with fourteen (14) staff providing care and supervision. Director and LPA viewed the area where C1 sustained the injury. LPA notes the area is within the CCC's outdoor playground and the surface surrounding the play structure is cushioned. LPA notes the play structure is age-appropriate. Director informed LPA that the incident occurred at 5:00 PM when C1 was walking down the steps of the play structure. C1 then lost their balance and hit their forehead on the play structure\342\200\231s step, resulting in a \302\274 inch cut on C1\342\200\231s forehead. S1 immediately responded to C1\342\200\231s injuries and informed the Director of the incident. After basic first aid was provided, C1\342\200\231s parents were immediately notified and C1 was picked up at approximately 5:15 PM. LPA was informed that C1\342\200\231s injury required two stitches and C1 returned to the CCC the following day, 02/04/2026. LPA concluded that C1 was engaged in normal activities and there was no inherent hazard. LPA confirms the Director\342\200\231s and S1\342\200\231s account of the incident corroborates the UIR submitted to Community Care Licensing (CCLD). To date, C1 is currently attending the CCC and has no prolonged issues related to the injury. Continued 809-C Based on observations and interviews with the Director, it is determined neither the equipment at the CCC nor the actions of the CCC's staff contributed to C1's incident. LPA best categorized the incident as an unfortunate accident. No deficiencies were cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the facility representative, Nicola Adams.
