What the inspector wrote

On 02/17/2026, Licensing Program Analyst (LPA) Shane Loftus conducted an unannounced Case Management inspection of the above-mentioned Child Care Center (CCC) to follow up on an Unusual Incident Report (UIR) received by the Department on 02/06/2026. Specifically, a child in care (C1) had a rock stuck in their nose, resulting in a nosebleed. LPA met with CCC Director Nicola Adams and explained the nature of the inspection. LPA notes sixty-five (65) children are on site along with fourteen (14) staff providing care and supervision. Director and LPA viewed the area where the incident occurred. LPA was informed that the rock may have come from an area of the play yard that was exposed and has since been repaired. Director informed LPA that the incident occurred when S2 noticed C1 was picking their nose and subsequently noticed their nose was bleeding. S2 then notified S1 of the situation and S1 cleaned C1\342\200\231s nose. S1 and S2 state they did not notice a rock in C1\342\200\231s nose and were not informed by C1 at the time of the incident. Director informed LPA that the CCC was only made aware of a rock in C1\342\200\231s nose the following day, 02/05/2026 by C1\342\200\231s parent. Director called Community Care Licensing Division (CCLD) on 02/05/2026 to provide the UIR. Director left a voicemail for the Officer on Duty and emailed the LIC 624 on 02/06/2026. Based on observations and interviews, it is determined that the CCC staff\342\200\231s actions and play area did not contribute to C1\342\200\231s incident. LPA determined that the CCC did not report the incident to CCLD within 24 hours because they were unaware of the extent of the incident until 02/05/2026, which is when the CCC called CCLD. LPA best categorized the incident as an unfortunate accident. No deficiencies were cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the facility representative, Nicola Adams.
