What the inspector wrote

This is a change of location. Previous facility number 426215565 . On May 27, 2026 Licensing Program Analyst (LPA) Giovani Gonzalez conducted an announced Prelicensing/Change of Location inspection at the above mentioned residence. LPA met with applicant Edis Chacon and informed them the purpose of the inspection. At the time of the inspection no children were present. Applicant has the intention to provide care for children ages 3 months to 9 years old and to have operating hours of Monday through Friday from 8AM - 5PM. LPA in the company of the applicant toured the interior and exterior of the residence in its entirety. LPA notes the home is a 2 story condo consisting of 3 bedrooms, 2.5 bathrooms, living room, kitchen/dining area, backyard and unattached garage. LPA notes the home's living room, half bathroom and backyard will be used for child care purposes. LPA observed the interior of the home to be clean and orderly. LPA observed the living room to have age appropriate equipment for children. LPA observed the living room to be free of hazards. LPA observed the kitchen/dining area to be accessible. LPA observed all lower kitchen cabinets to be secured. LPA observed the sharps to be located in an elevated surface beyond the reach of children. LPA observed the stairs to be secured with a child proof gate. LPA notes all bedrooms and 2 bathrooms are upstairs and are inaccessible to children in care. LPA observed the half bathroom to be used for child care purposes to be clean and free of hazards. LPA observed a regulation fire extinguisher (2A10BC) in the home which was purchased on 10/10/25. LPA reminded applicant of the responsibility to service or purchase a regulation fire extinguisher annually. The home has multiple smoke and carbon monoxide detectors. A combination smoke/ carbon monoxide detector was tested at 9:24AM and found to be in working order. CONTINUED LIC 809C LPA observed the outdoor area to be completely enclosed. LPA observed the outdoor area to have plenty of shade and space for children's activities. LPA observed the yard to be free of hazards. LPA observed the garage door to be secured and inaccessible to children. LPA's record review revealed applicants' Mandated Reporter training was completed on 7/17/24, (expiration 7/16/26) and Pediatric CPR/First Aid (EMSA approved) was completed on 7/8/24 (expiration 7/8/26). LPA reminded applicant of obligation to maintain current training and certifications. LPA reviewed applicants' control of property document (Mortgage). Applicant does not have liability insurance for the home. LPA provided applicant with Affidavit Regarding Liability Insurance form (LIC 282). Applicant informed LPA no firearms or ammunition are on site. Fire Clearance was granted on 5/19/2026 by the Ventura City Fire Department. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The applicant provided proof of control of property. This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. Entrance Checklist was provided to the applicant. CONTINUED LIC809C LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. On this date, 5/27/26, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given to applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. The home meets Title 22 Division 12 requirements for a Large FCCH License. Effective date of license will be noted as the present, 5/27/2026. Exit interview conducted and report was reviewed with the applicant Edis Chacon.
