Inspector’s narrative

What the inspector wrote

An exit interview was conducted with the facility representative. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA Scott informed facility representative, Carissa Crain, that this report dated April 29, 2024, documents one Type A citation. Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Scott informed the facility representative to provide a copy of this licensing report dated, April 29, 2024, that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

    (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance.This requirement was not met as evidenced by: LPA reviewed sign in and out sheets, interviewed staff regarding schedules, and determined facility has been over ratio at least on one occassion (4/29/24).This poses an immediate health, safety or personal rights risk to children in care.
