What the inspector wrote

On October 24, 2024 Licensing Program Analysts (LPA) Jennie Tedlos met with Program Director, Kelly Twibell (D1) to follow up on an Unusual Incident Report (UIR) submitted to Community Care Licensing on October 22, 2024. On October 22, 2024 an incident occurred, during outside dismissal time, a child (C1) exited a classroom gate unsupervised, then walked out of the entrance gate which was left open by a guardian entering the facility. The child continued to walk down the facility's sidewalk adjacent to the parking lot. The guardian notified a nearby teacher (T1) that the child exited the facility. T1 ran to the child and stayed with them until D1 was able to bring the child back to their class. LPA conducted interviews that support the reported incident. Per D1, immediately following the incident, corrective actions have taken place by the facility, guardians have been notified and reminded of gate safety, and staff training has been scheduled regarding supervision. The facility has also put a carabiner clip on the classroom gate to prevent children from opening it and have created a new strategy for outside dismissals. A deficiency is being cited based on LPA observation, interviews conducted, and record review in accordance with the California Code of Regulations, Title 22, see LIC809D. A Type A violation regarding Supervision has been assessed. Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 809D in each child's files. An exit interview was conducted and Plans of Correction were reviewed and developed with the Director, Kelly Twibell. A copy of this report and appeal rights were discussed and left with the Director. A notice of site visit was provided and posted by LPA and must remain posted for 30 days.

This is considered an immediate health, safety, or personal rights risk to children in care.
