What the inspector wrote

The following Title 22 Deficiency is being cited on the subsequent LIC9099-D page. Upon receipt of Type A citations, the Center Manager shall post and provide copies of the LIC9099-D for parents/guardians of children in care and for parents/guardians of newly enrolled children for the next 12 months. Center Manager must also keep the signed LIC9224, Acknowledging Receipt of LIC9099-D in each child's file. In addition to be being cited today, a civil penalty is being assessed as this is a repeat violation for a lack of supervision within the last 12 months. Appeal Rights were provided. A notice of site visit was provided by LPA Mayberry to Center Manager as the facility is not currently opened.

(1) No child(ren) shall be left without supervision. . .Supervision shall include visual observation. This was evidenced by the following: on May 16, 2022, a lack of supervision lead to an opportunity allowing several children to go under a play structure and urinate in cups. This poses and immediate risk to the health and safety of children in care.
