Inspector’s narrative

What the inspector wrote

...positive reinforcement, redirection, bonding activities, increased supervision or shadowing, and discussions with Child 1 about emotions. However, according to staff, parent, and children\342\200\231s interviews, Child 1\342\200\231s behaviors have continued, resulting in an unsafe environment to other children in care. Based on interviews, file review, and observations conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Type-B Deficiency is cited on a subsequent 9099-D page. An exit interview was conducted, and the report was reviewed with the Lead Teacher, Alex Lu. LPA provided Lead Teacher Alex Lu with Appeal Rights. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

LPA learned through staff and children interviews that Child 1 has been using verbal insults and physical violence towards other children in care. This poses an immediate health & safety and personal rights risk to children in care.

This poses an immediate health, safety, and personal rights risk to children in care.
