What the inspector wrote

Based on interviews and file reviews conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Type-A Deficiency is cited on a subsequent 9099-D page. Upon receipt of Type A citations, the facility shall post and provide copies of the LIC 9099D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 9099D in each child's files. An exit interview was conducted, and the report was reviewed with the Lead Teacher, Alex Lu. ***This report is amended from an original report dated 8/29/24.*** ****Teacher, Zoe Smith, signed today's amended report on 9/4/24.*****

    101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. LPA learned through staff and children interviews that Child 1 has been using verbal insults and physical violence towards other children in care. This poses an immediate health & safety and personal rights risk to children in care.

This poses a potential health & safety and personal rights risk to children in care.

This poses an immediate health, safety, and personal rights risk to children in care.
