On 09/08/25, Licensing Program Analyst Jennie Tedlos (LPA) conducted a case management inspection to verify corrections of a deficiency cited on 08/11/25. LPA arrived at the facility and was met by Director Susan Schuster (D1). LPA toured the facility and observed 10 preschool aged children being supervised 2 staff members. On 08/11/25, the facility was cited a Type B deficiency for toys and play equipment with sharp and pointed parts. The facility was also cited a Type B deficiency for lack of cushioning underneath a tall slide. The facility has fixed the toys and play equipment with sharp and pointed parts and provided sufficient cushioning underneath the tall slide. The deficiencies were cleared by today's field visit. A Proof of Correction letter was provided to D1. No deficiencies were cited today in the areas that were evaluated. An exit interview was conducted and the report was reviewed with Director Susan Schuster. Licensee Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted for 30 days.
