What the inspector wrote

Based on the information obtained throughout the course of this investigation, the above allegations could not be substantiated or dismissed. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the finding is UNSUBSTANTIATED. An exit interview was conducted and report was reviewed with the Director, Lauren Garcia. A notice of site visit was given and must remain posted for 30 days. Based on the information obtained throughout the course of the investigation, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. Title 22 Regulations are being cited on the attached 9099-D pages. LPA informed the Director, Lauren Garcia, that this report dated 11/04/2022 documents one Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care. LPA informed the Director to provide a copy of this licensing report dated 11/04/2022 that documents the Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed LIC 9224, Acknowledgement of Receipt of Licensing Reports, must be placed in each child's file for verification. An exit interview was conducted and report was reviewed with the Director, Lauren Garcia. A notice of site visit was given and must remain posted for 30 days.
