(a) The licensee shall provide care and supervision... (1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews, the facility did not comply with the section cited above where one child had exited the classroom without staff knowledge or supervision of any staff, which poses an immediate health, safety or personal rights risk to persons in care.
