What the inspector wrote

An unannounced case management inspection was conducted today on 4/17/25 at 2:30 pm by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with Licensee Kayla Jones in response to an Unusual Incident Report received by the Department on 4/17/25. On 4/11/25 a child in care (C1) rolled off her bed as they were preparing for nap time and injured her left elbow. The Licensee was interviewed on 4/17/25 at 2:30pm and stated that on 4/11/25 at 11:00 am a child (C1) fell off her cot and hurt their shoulder. There were five children present with one staff member during the incident. Licensee attempted to comfort child, but they were inconsolable. Licensee called C1's parent and C1 was taken to receive medical care. C1's parent (P1) was interviewed on 4/17/25 and stated that they were getting a permanent cast for C1. P1 indicated that C1 fell from a mat that is approximately 1 and 1/2 inches off the ground. P1 stated that they did not feel there was a lack of supervision and they do not have any concerns for safety. P1 indicated that their child has a lot of energy and they felt it was just an unfortunate accident. During today’s inspection, the facility was toured inside and LPA inspected cots for safety. LPA determined the cots were age appropriate for 2-5 year olds. Photos of manufacturing guidelines were obtained. During today's visit three children and one staff member were in the facility. LPA observed facility was in ratio and no Title 22 violations were observed. There were no deficiencies cited during today’s inspection. Based on interviews and inspection it was determined there were no violations or safety concerns regarding the incident. Children in care were appropriately supervised. Exit interview conducted and report was reviewed with the facility representative Kayla Jones. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days.
