What the inspector wrote

On 4/14/26 at 11:50am Licensing Program Analyst (LPA), Tammy Dutra conducted a case management facility inspection and met with licensee Crystal Miner. This inspection was in response to an application for a change of location that was received by the Department 4/2/26. The fire clearance was granted on 4/13/26. The LPA toured the facility's indoor and outdoor areas. The home is a five bedroom and three bath two story home. The following areas will be off limits to children: entire upstairs, one downstairs bedroom, two garages and outdoor shed. These areas have been made inaccessible by means of gates and locks. A copy of the staffing ratio requirements was provided and discussed with the licensee. The licensee acknowledged she understood the ratio requirements. The licensee understands that an assistant provider under the age of 18 cannot be left alone without an adult on the premises. Chemicals and cleaning agents are locked in the garage and the outdoor shed which has three locks (two top locks) and a padlock. LPA advised any poisons present in the home be locked in an off limit area. The home is equipped with a working smoke detector and fire extinguisher rated at least 2-A-10-BC. The children will use the backyard as the outdoor play area. The backyard is completely fenced. There is no pool, spa, pond, fountain, or any other body of water on the premises. The LPA reviewed the ratio's for a large license and the licensee acknowledged she understood the ratio requirements. Because the licensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). The LPA also reviewed the Safe Sleep requirements with provider and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. On this date, 4/3/26, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Licensee has CPR/First Aid certification which expired 3/11/26. Licensee registered for the new course which will be completed 4/21/26. Mandated reporter certificate expires 4/3/27. Assistant has current mandated reporter certificate and pediatric CPR/ First Aid which expires 8/20/27. Licensee has met requirements to grant a change of location provisional license. LPA will process this location change and mail an updated license to reflect this change of location effective today . Once Licensee completes Pediatric CPR/ First Aid Certificate and submits copy to CCL LPA will update provisional license. An exit interview was conducted and report was reviewed with licensee, Crystal Miner. Appeal rights were provided. Notice of Site visit was provided and shall remain posted for 30 days. Failure to follow posting requirements will result in a civil penalty of $100.
