What the inspector wrote

1418.81(d)(4) Health & Safety Code 1418 (d) If return to the community is part of the care plan, the facility shall provide to the resident or responsible party and document in the care plan the information concerning services and resources in the community. That information may include information concerning: (4) Other resources or services in the community available to support return to the community. The facility failed to ensure all available remedies for a safe and orderly discharge were in place and failed to address all discharge needs in a comprehensive, post-discharge plan, for Resident 1. This failure had the potential for Resident 1's failure to thrive in the community, resulting in possible necessary readmission to a healthcare facility. During an interview on 7/12/21 at 9:30 a.m., with the facility SSD (Social Services Director), the SSD stated she was not employed by the facility at the time of Resident 1's 30-Day Discharge Notice. She was able to pull the files and found the former SSD was the person who wrote the paperwork and documentation for this notice and for this potential discharge. Resident 1 was given the notice because he had failed to pay his bill for his Share of Costs, which had accumulated to $38,385.00, since his admission on 10/04/16. Documentation review on 7/12/21, revealed a Progress Note, dated 12/15/17, wherein the former SSD documented she worked extensively with Resident 1, completing applications to find him appropriate care in the community. Resident 1 was taken to see several apartments, which he turned down for various reasons. Resident 1 received his 30-Day Notice on 3/26/19, which indicated he would be transferred to a local hotel. According to a Discharge Appeal Decision, dated 4/16/19, it indicated the facility failed to demonstrate the discharge to the local hotel would be safe and orderly; prior to identifying the hotel as Resident 1's post-discharge destination: The facility did not visit the hotel, to ensure it could accommodate Resident 1's needs; the facility failed coordinate with the hotel for a daily rate, thereby discovering Resident 1 could only afford nine-days' tenancy before his funds would run out; the facility also failed to develop a Discharge Plan of Care, addressing alternative resources to reduce factors leading to preventable readmission or other placement, in the event the hotel placement did not work out. Facility Policy, entitled, "Discharge A Resident," updated 6/2017, indicated: "It is the policy of this facility to provide a safe departure.... will assist in a comfortable adaptation to .... a new environment...." The above violation had a direct or immediate relationship to the health, safety or security of Resident 1.
