F 0921 Level of Harm - Minimal harm or potential for actual harm Residents Affected - Some Make sure that the nursing home area is safe, easy to use, clean and comfortable for residents, staff and the public. **NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, interview, and record review, the facility failed to provide a safe and comfortable environment, when multiple residents complained of cold temperature and the residents' rooms temperatures were below the requirement. In addition, the facility failed to act in a timely manner when the boiler (a central component of heating system that provides warmth to the building) was not functioning for a census of 98. These failures resulted in residents' discomfort, difficulties with sleeping and had the potential to negatively impact the residents' quality of life.Findings: An investigation of the complaint that alleged the facility was too cold and the heater was not working properly was conducted on 11/22/25, commencing at 7:30 a.m. The temperature outside registered 42 degrees F (Fahrenheit, unit of measurement) at 7:30 a.m. During an observation on 11/22/25 at 7:35 a.m., five residents were observed in the dining room. A female resident sitting next to the table had a warm sweater and pants on. The resident kept her hands tucked under the warm scarf which was lowered all the way on resident's chest area. Resident stated the room was cold and she was trying to warm up her hands.During an observation and interview on 11/22/25 at 7:39 a.m., Resident 1 was at the table holding a cup of coffee. The resident was dressed in short sleeves t-shirt and was visibly shivering. Resident 1 stated the facility had been cold for a while, especially at night. Resident 1 added, I asked for 2 blankets last night because I could not fall asleep. Resident 1 added, I have seen that some family members bring extra blankets. Three other residents dressed in long sleeve sweaters and pants were napping in their wheelchairs and were unavailable for interview. Two vents in the ceiling were blowing cold air and the thermostat on the wall registered at 61 degrees F.During an interview on 11/22/25 at 7:45 a.m., Staff (ST 3) stated the facility was super cold and added that it was colder than usual. ST 3 was dressed in long sleeve warm sweater and stated, I move a lot and still cold. ST 3 stated that residents complained of being cold, especially at night and every morning they asked for extra blankets. ST 3 stated the facility had a problem with heater not working properly. ST 3 stated the facility administration were aware that heater was not working and added, looks like it was not fixed.During an observation on 11/22/25 at 8 a.m., Resident 2 was observed in bed dressed in warm pajamas and covered with 2 blankets. Resident 2 stated, We are freezing, we have been cold all night. It's hard to sleep when you are so cold. Resident 2 stated she was not sure if heater was working. Resident 2 pointed to the thermostat on the wall and stated, That thermostat is not working. Every time I ask someone to increase the temperature, they say they can't, the thermostat either locked or they don't know how to increase temperature. Resident 2 waved to the passing staff on the hall and asked to check if the thermostat was on. Resident 2 told the staff, We are very cold.During an interview on 11/22/25 at 8:05 a.m., ST 3 stated, It is cold, I don't think the heater is working properly. ST 3 explained that facility has been cold for a week or longer, since the rain started, and nights became colder. ST 3 explained, When I try to adjust the thermostat, it doesn't seem helping much, still cold and no heat coming from the vent above. ST 3 stated, I talked to one of the maintenance and was told he will look into it. I don't think he was able to adjust it . still no warm air coming out. The thermostat on the wall showed 65 degrees F.During an observation and interview on 11/22/25 at 8:10 a.m., Resident 3 was observed in bed covered with three warm blankets up to his chin. Resident 3 stated, Very cold morning, colder at night. Not sure if heater is on, I don't feel like its blowing warm air. Resident 3 added, Yes, staff know that its cold. When I ask why it's cold, they say they don't know. What can I do . just ask for extra blankets. The temperature on the thermostat showed 65 degrees F and no warm air was coming from the vent above.During an observation and interview on 11/22/25 at 8:17 a.m., Resident 4 was observed leaving the dining room and walking in the hall pushing her walker. Resident 4 was wearing full length warm coat, sweatshirt and jeans. Resident 4 stated, It's very cold, feels like the vent is blowing cold air. Resident 4 stated, I had three blankets at night to keep me warm and bring this one with me. Resident 4 pointed to her walker where a heavy fur folded blanket was placed. Resident 4 added, It's cold in the dining room so I want to make sure I have something to put on my shoulders if I get cold.Not sure what is going on but it's very uncomfortable.During an observation and interview on 11/22/25 at 8:25 a.m., Resident 5 was sitting in wheelchair and eating his breakfast. The resident was dressed in long sleeved sweatshirt and pants and had two warm blankets on the bed. Resident 5 stated, Very cold at night. Uncomfortable, hard to sleep when it's cold. [I] Asked for another blanket and was able to fall asleep. I don't think the heater is working. Resident 5's roommate was observed in bed in a fetal position with a warm blanket covering his head. The thermostat on the wall showed 65 degrees F and no noise and warm air was coming out from the vent on the ceiling.During an observation and interview on 11/22/25 at 8:30 a.m., Resident 6 was observed sitting in her bed eating breakfast. The resident had three warm blankets covering her from waist down and a thick throw on her chest area. Resident 6 stated, The room is very cold, and even colder at night.I do not like to be cold. Freezing cold at night. The resident pointed to the thermostat and added, It's not working. I told multiple people, my nurses, aides, whoever comes here. Resident 6 stated, I was either ignored or told there was nothing that could be done when I asked to have the thermostat adjusted.They say they do not know how to adjust and will ask maintenance to check on it. I have these warm blankets and do not say any more about the thermostat. The thermostat on the wall showed 65 degrees F and no warm air was coming out from the vent on the ceiling.During an observation and interview on 11/22/5 at 8:38 a.m., Resident 7 was observed lying in her bed covered with two warm blankets. Resident 7 stated, It had been very cold in my room, usually during the mornings and all throughout the night. I had informed staff of the uncomfortable temperature, but nothing is done. I don't think they know how to adjust that thermostat and don't even know if the heater is working. During an interview on 11/22/25 at 8:45 a.m., ST 2 validated that the facility was cold. ST 2 stated that earlier in the morning two residents complained that it was too cold in the dining room and I was not able to adjust the settings on the thermostat. When ST 2 was asked if it was possible to increase the temperature on thermostats, ST 2 replied, Some of the rooms I am able to adjust. ST 2 stated that thermostat in room [ROOM NUMBER] was off. When ST 2 turned the thermostat on, the thermostat started making noise, but no heat was coming out of the vent. ST 2 stated the maintenance staff were informed of issues with cold rooms and added, Not sure if they checked if the heater is working properly.During an observation and interview on 11/22/25 at 8:55 a.m., Resident 8 was in bed dressed in street clothes and covered with two blankets. Another warm blanket was observed folded on the bed. F 0921 Level of Harm - Minimal harm or potential for actual harm Residents Affected - Some sitting next to her. Resident 8 stated, Very cold, especially at night. I think that inside is not much warmer than outside temperature. Lying uncomfortable until I fall asleep, but when I wake up I wouldn't go back to sleep because I'm so cold. The thermostat on the wall showed 65 degrees F. Resident 8's FM stated, It's been going for forever. It was cold when [Resident 8's name] was in previous room. I asked charge nurse several times, complained of room being cold and nothing was done. Every time I'd report, they would say, I'll talk to maintenance and I have never seen maintenance looking at that thermostat trying to adjust it. Seems like nobody cares.During an observation and interview on 11/22/25 at 9:05 a.m., Resident 9 was sitting at the edge of her bed, bending forward. The resident was dressed in a long warm cardigan, fleece long-sleeved sweater, and pants. Resident 9 stated, Its cold, cold at night and cold in the morning. I don't like it to be that cold. I have 2 warm blankets at night, but I sleep dressed in my clothes to be warm. Resident 9 stated that she complained to multiple staff about cold room and asked to check the thermostat. Resident 9 added, I don't think the heater is on.usually it makes noise when it's working. The thermostat on the wall showed 65 degrees F and no warm air was coming from the vent.During an observation on 11/22/25 at 9:35 a.m., Resident 11 was observed sleeping in his bed curved in fetal position. The resident had warm sweater with hood on, vest with fur lining on, sweatpants and warm socks and covered with warm blanket. The thermostat on the wall showed 65 degrees F and no warm air was coming from the vent.During an interview on 11/22/25 at 9:38 a.m., ST 5 was observed charting in the hallway. ST 5 was wearing long sleeved warm sweater. ST 5 stated, It's cold outside and inside. ST 5 stated that when residents complained that they were cold, she would bring them extra blankets. When ST 5 was asked to check the thermostat in Resident 11's room, ST 5 replied, Not sure if it works or not; I don't know how they work and I don't know how to adjust them.During an interview on 11/22/25 at 9:40 a.m., the Director of Nursing (DON) stated the heater was turned on and should be working properly. The DON stated, I have not heard any concerns with heater not working and I have not heard any complaints from residents that facility is cold. During a walk in the hallway by the dining room, the DON acknowledged that the cold air was blowing from the vents and it was cold in hallway. The DON was asked to check the dining room and the DON validated that the room was cold and the thermostat in dining room showed 61 degrees F.During an observation on 11/22/25 at 9:45 a.m., Resident 12 was observed in wheelchair leaving the dining room, accompanied by family member. The resident had warm sweater and a hat on. Resident 12 stated, Freezing in the dining room, my hands are too cold. Very uncomfortable. Resident 12's FM stated that since the weather changed, [it]started raining and became colder outside, the facility had been cold. The FM stated she talked to the maintenance a few days ago and complained that her sister and other residents were cold. The FM stated that the maintenance staff told her he will turn the heater on. The FM added, Seems like it had not been turned on. It's cold here.During an observation and interview on 11/22/25 at 9:55 a.m., Resident 8 was visited in her room accompanied by DON. The resident was covered with three warm blankets and stated, It's okay now but it is very cold at night. I was freezing last night and asked my husband to bring another warm blanket from home. When the DON asked Resident 8 if she reported to the staff that her room was cold, the resident replied, Yes of course I did. I reported to multiple staff, nurses and CNA's and nothing was done except offering another blanket.On 11/22/25 at 10:10 a.m., when the Department brought up the residents' complaints of being cold and heater not working properly to the Administrator (ADM), the ADM replied, Not aware of any issues with heater and facility being cold.During an interview on 11/22/25 at 10:20 a.m., the Plant Operations Assistant (POA) stated, No issue with heater, and we had no complaints that the facility is cold in the last three weeks. F 0921 Level of Harm - Minimal harm or potential for actual harm Residents Affected - Some temperatures in residents' rooms daily in the morning and provided a log of daily recordings for the week of 11/17/25. According to the log, the lowest temperature recorded on 11/21/25 was 69 degrees F and the highest was 73 degrees F. The POA was informed that multiple residents complained of facility being cold, and the POA stated, The heater is on but our boiler went out so there is no warm air coming in. The POA was not able to provide the exact date when the boiler stopped working by stating, Sometimes last week. The POA stated that a few days ago the technician from heating and ventilation company checked the boiler and told the facility that he needed to order some parts to fix the boiler. The POA stated that the facility planned to fix the boiler and heater in two days. When the ADM (present during the interview) was asked what was done for facility's vulnerable residents in the meantime when the heater was not working, the ADM did not provide an answer.During a continued interview on 11/22/25 commencing at 10:20 a.m., with ADM and POA, the Department brought up a concern that staff were not able to identify if thermostats in multiple rooms were working and were on and to adjust them. The Department Informed the POA and ADM that the thermostat in room [ROOM NUMBER] was making loud noises and no warm air coming out after being turned on for over an hour. The POA explained, No warm air because the boiler is not working. When the Department clarified that the heater was not functional due to broken boiler, the POA replied yes. Upon reviewing the temperature log for the week of 11/17/25 provided by the POA earlier, the POA was asked how the maintenance got the temperature to be at 69 to 73 degrees F range in 8 rooms 24 hours prior, when the boiler was identified not to be working, the POA was not able to explain. On 11/22/25 at 10:30 a.m., a tour of the residents' rooms and temperatures checking was conducted with ADM and POA and the following was observed:1. room [ROOM NUMBER] thermostat indicated between 60- and 65-degrees F. The POA explained, I don't think the thermostat is on, but it won't blow warm air because of the boiler not working. Once we turn the boiler on, it will start blowing warm air into residents' rooms. The POA checked temperature by pointing laser gun thermometer on the wall above resident's head and it showed 64 degrees F. The ADM and POA acknowledged that the temperature was too low.2. The thermostat in room [ROOM NUMBER] was turned off and registered at 65 degrees F when the POA checked the temperature with laser gun thermometer on the wall. The POA stated that air temperature in residents' rooms should be ranging between 71 degrees to 81 F. The ADM and POA validated that 64 degrees F was unacceptable and too cold.3. The thermostat in room [ROOM NUMBER] indicated 45 degrees F. The POA stated, I think its broken. When the POA checked the temperature with laser gun, it showed 66 degrees F. The POA acknowledged that the temperature was below the requirement.4. The thermostat in room [ROOM NUMBER] indicated 65 degrees F and when rechecked by POA with laser gun, the temperature registered as 67 degree F.During a continued tour of the facility on 11/22/25 at 10:40 a.m., Resident 6's room thermostat indicated 65 degrees but it was turned off. Resident 6 stated, Its chilly now with couple of blankets on me, but very cold at night. Freezing cold. Please, do something, its uncomfortable. The ADM asked if the resident reported to the staff that her room was cold and Resident 6 replied, Staff is aware, but they say they don't know how to fix it.During an interview with ADM on 11/22/25 at 10:50 a.m., the ADM confirmed that multiple rooms that were checked for temperatures indicated temperatures were low, unsafe and uncomfortable and were below the requirements.
