F 0880 Provide and implement an infection prevention and control program. Level of Harm - Minimal harm or potential for actual harm Based on observation, interview, and record review, the facility failed to provide infection prevention and control measures to prevent the possible spread of Clostridioides difficile (C. diff- a bacterium causing severe, often hospital-acquired, diarrhea, and colitis, frequently triggered by antibiotics disrupting normal gut flora) for a census of 93; when staff failed to perform appropriate hand hygiene (act of cleaning one's hands to remove harmful and unwanted substances) between residents.This failure put residents at risk of contracting C. diff, with the potential of causing illness or death.Findings:A review of Resident 1's admission RECORD, indicated Resident 1 was admitted to the facility in February of 2026 with diagnoses that included, .ENTEROCOLITIS [inflammation of the digestive tract] DUE TO CLOSTRIDIUM DIFFICILE, NOT SPECIFIED AS RECURRENT.URINARY TRACT INFECTION, SITE NOT SPECIFIED.A review of Resident 1's orders indicated, .Resident is on contact precaution. Use gown and gloves when entering the room. Ensure proper hand hygiene before and after contact with the resident or their environment .During an interview on 2/20/26, at 10:40 a.m., with the Infection Preventionist (IP), the IP stated, Resident 1 was recently admitted with a C. diff infection and was placed in Hall 400 with contact precautions (infection control steps used in healthcare settings to prevent the spread of germs that are passed by direct contact with a patient or their environment [surfaces, equipment]). During an observation on 12/20/26, at 12:38 p.m., in Hall 400, Certified Nursing Assistant (CNA) 1 was observed entering Resident 1's room with the appropriate Personal Protective Equipment (PPE- protective clothing, helmets, goggles, or other garments/gear designed to protect the wearer's body from injury or infection in industrial, medical, or emergency settings).During an observation on 12/20/26, at 12:40 p.m., in Hall 400, CNA 1 exited Resident 1's room, removed her PPE, performed hand hygiene with alcohol based gel located outside the room, and walked down the hall and entered another resident's room. During an interview on 12/20/26, at 12:42 p.m., with CNA 1, CNA 1 confirmed she performed hand hygiene using an alcohol-based sanitizing gel located outside the Resident 1's room. CNA 1 stated Resident 1 had C. diff and she had done appropriate hand hygiene before moving on to the next resident. During an interview on 12/20/26, at 12:59 p.m., with the IP, the IP stated she expected the staff to use proper contact precautions when caring for a resident with C. diff. The IP nurse further stated the proper hand hygiene for C. diff was to use soap and water. CNA 1 explained that alcohol-based sanitizing gel was not enough as it would not kill the C. diff bacteria. During an interview on 12/20/26, at 1:07 p.m., with the Director of Nursing (DON), the DON stated she expected staff to perform hand hygiene with soap and water when providing care to a C. diff resident. The DON further stated the risk was the spread of infection to other residents. The DON explained the importance of handwashing with soap and water was so that the C. diff infection could not spread.Review of an undated facility document titled, Clostridium Difficile, indicated, .5. Steps toward prevention and early intervention include.d. frequent hand washing with soap and water by staff and residents.Review of an online document posted by the Centers of Disease Control and Prevention (CDC) titled, Preventing C.
