F 0925 Make sure there is a pest control program to prevent/deal with mice, insects, or other pests. Level of Harm - Minimal harm or potential for actual harm **NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, interview, and record review, the facility failed to provide adequate pest control services for bed bugs when reviewed for physical environment. This failure has the potential to affect all 107 residents residing in the facility.Findings Include:On 1/6/26 at 1:58 PM, room [ROOM NUMBER] was observed, no residents reside in this room. Approximately seven dead bed bugs were noted on a glue board, one live bed bug noted on the mattress by the door.On 1/8/26 at 8:40 AM, room [ROOM NUMBER] was observed, this room does not house any residents, one live bed bug observed on the baseboard and one live bed bug on the mattress sheet. On 1/8/26 at 8:43 AM, room [ROOM NUMBER] was observed, this room does not house any residents, several dead bed bugs observed on the baseboards.On 1/6/26 at 10:14 AM, V6, Maintenance Director, stated the facility has not had any residents with bed bugs for a while. V6 stated the facility sprays for bed bugs every Monday, Wednesday, and Friday. V6 stated the facility does have bait sticky traps out for bed bugs and have found some bugs on the traps but have not had any live bed bug sighting. V6 stated he is unsure if the bed bugs that have been found on the sticky traps are coming from the walls or not. V6 stated no residents have had any bed bugs on them or any resident rooms including curtains and bed linens. V6 stated if a resident's room is found to have bugs, the resident is moved out of that room, and everything is heat treated and washed. V6 stated residents are kept out of that room for at least a month and that room is on a list to be watched. V6 stated if a resident complains about bed bugs, and no bugs are found, that room will be put on the list and sprayed every Monday, Wednesday, and Friday. V6 stated residents will only be moved out of the room if they have active bed bugs in them and all residents will be moved out of the room. On 1/6/26 at 1:50PM, V1, Administrator, stated she has been at the facility since November 2025 and at that time, they were finishing up treatment for bed bugs, she is not aware of any live sightings since then. V1 stated she thinks they found a dead bed bug downstairs on the 500 hall in December and pest control came and sprayed. V1 stated she believed room [ROOM NUMBER] was also treated. On 1/8/26 at 9:00 AM, V13, Pest Control Service Technician, stated the facility has bed bug issues but not as bad has they were. V13 stated bed bugs are hitch hikers and residents/staff can transfer them from one unit to another. Today I treated rooms [ROOM NUMBERS] as a follow up, there was activity in 512 last week because there was dead bed bugs identified in that room. I did not see any active bed bugs today and the tenet said it is getting better. Once an initial visit is conducted, we do two more follow up visits on top of the regular visits to see how the treatment is working. Bed bugs hide under the baseboards. There are two options for treatment of bed bugs, chemical spray or heat treatment, this facility is using the spray treatment. A lot of facilities opt for that treatment because the heat treatment is costly. The chemical treatment requires more treatments, where the heat treatment is a one and done solution. Once the room is sprayed, the bed bug will go through the spray and pick up its spores and transfer it back to the colony so it can take four - seven days for it to be effective and Residents Affected - Many (continued on next page) Any deficiency statement ending with an asterisk (*) denotes a deficiency which the institution may be excused from correcting providing it is determined that other safeguards provide sufficient protection to the patients. (See instructions.) Except for nursing homes, the findings stated above are disclosable 90 days following the date of survey whether or not a plan of correction is provided. For nursing homes, the above findings and plans of correction are disclosable 14 days following the date these documents are made available to the facility. If deficiencies are cited, an approved plan of correction is requisite to continued program participation. As maintenance, the facility can use torpedoes and glue boards to monitor for activity. V13 stated prior to a visit for active bed bugs, the facility has to strip everything, bag all linen, including the curtains, wash and dry it. There can't be anything in the closets or against the baseboards when the room is sprayed. Once the room is sprayed the resident has to remain out of their room for 4 hours and then they can return. V13 stated once treatment is complete, if the bed bugs return it is because they are being brought back in by staff, residents, visitors, etc. On 1/8/26 at 9:30 AM, V16, Housekeeping, stated there was a problem with bed bugs but she hasn't seen any recently. V16 stated once bed bugs are identified, she notifies maintenance and housekeeping will strip the entire room, including all linen, curtains, etc. Take the linen to laundry where it is placed in the dryer first to kill them, then they are washed and dried again before being sent back to the room.On 1/8/26 at 9:45 AM, V19, CNA (Certified Nursing Assistant), stated she had seen bed bugs in some of the 100/200 hall resident rooms, unsure of when she saw them last, but not recently because they have treated for them. V19 stated when she has seen bed bugs, she reports it to maintenance and the nurse because it can cause irritation to the resident's skin. On 1/8/26 at 9:50 AM, V21, CNA, stated she is not aware of any current bed bug problems, they have come and sprayed, and she hasn't seen any. The Pest Control Quote, dated 11/27/26, documents services for bed bugs, conventional treatment, were requested.The Pest Control Service Inspection Report, dated 12/11/25, documents V13, Pest Control Service Technician talked with V6, Maintenance Director, about coming out on 12/15/25 for bed bug work.The Bed Bug Policy, with a revision date of 9/2017, documents, in part, the following: The policy is to provide guidance on how to handle bed bugs. If a resident comes from home, they will immediately be placed in a bag and sent to laundry where they will be washed at a temperature greater than 115 degrees, enough to kill the bed bugs. If bed bugs are found in a resident room the following will occur: all resident clothing and cloth belongings will be bagged and sent to laundry and washed at a temperature greater than 115 degrees; the pest control company will be notified to treat the room, as well as adjoining rooms as necessary; the room will be vacated for a thorough cleaning; the mattress will be removed and either disposed of or sent back to the rental company; the room will be thoroughly vacuumed including drapes, chairs, and crevices; the resident will receive a full body check and the physician will be notified of any red patches and that bed bugs were found and treatment will be ordered as determined by the physician; the infection control preventionist/nurse will be notified and will determine the prevention of transmission; reporting will be done to IDPH (Illinois Department of Public Health) after consultation to determine if it is a cluster of cases that may indicate a public health hazard.The Midnight Census Report, dated 1/5/26, documents there are 107 residents residing in the facility.
