What the inspector wrote

This survey cited 2 deficiencies. The full statement and the facility’s plan of correction follow, verbatim from the federal record.

F 0679 Provide activities to meet all resident's needs. Level of Harm - Minimal harm or potential for actual harm Based on review of facility documents, and resident and staff interviews, it was determined that the facility failed to provide an ongoing program of activities designed to meet the individual needs and interests for four of four residents reviewed (Residents 1, 2, 3 and 4).Findings include: In an interview with Resident 3 on February 2, 2026, at 11:05 AM, the resident stated activities are often cancelled because there are only two activity staff and there are often days with no activities at all. In an interview with Resident 2 on February 2, 2026, at 11:15 AM, the resident stated the activities used to be better than they are now, and there are not always activities to attend. Resident 2 also stated there are no alternative activities if they do not like an activity scheduled, and they are very concerned that the facility is cutting the activities. In an interview with Resident 1 on February 2, 2026, at 11:25 AM, the resident stated activities have been cancelled and now there are days without any activities at all, which bothers them. In an interview with Resident 4 on February 2, 2026, at 2:00 PM, the resident stated they stayed in bed this past weekend because there were no activities to attend. Resident 4 stated they like having something to do on the weekends. Review of the facility activity calendars for December 2025, and January 2026, revealed that in January there were no activities scheduled on the first, third, and fifth Saturday of the month, and the following Sundays listed only a single church service. Further review of the December 2025, and January 2026, activity calendars revealed a reduction in the number of activities available in January when compared to December, in addition to the removal of evening activities during the month of January. Interview with the Nursing Home Administrator on February 2, 2026, at 1:30 PM confirmed that the activity department had recently undergone staffing reductions, which caused the decrease in available activities. The above findings were reviewed with the Nursing Home Administrator and the Director of Nursing on February 2, 2026, at 3:00 PM. 28 Pa. For nursing homes, the above findings and plans of correction are disclosable 14 days following the date these documents are made available to the facility. F 0802 Level of Harm - Minimal harm or potential for actual harm Residents Affected - Many FORM CMS-2567 (02/99) Previous Versions Obsolete Provide sufficient support personnel to safely and effectively carry out the functions of the food and nutrition service. Based on observation, review of facility documents, and resident and staff interview, it was determined that the facility failed to provide sufficient staff to carry out the functions of food and nutrition services in the main kitchen for one of four units (University, Residents 1, 2 and 3).Findings include: Interview with Resident 3 on February 2, 2026, at 11:05 AM, revealed that the resident sometimes receives food served on Styrofoam for all three meals in a day. The resident stated they did not like Styrofoam because the food gets cold quickly. Interview with Resident 2 on February 2, 2026, at 11:15 AM, revealed that meals are served on Styrofoam often and it has happened recently, even in the last week. The resident stated, when it is a hot meal, it can be less hot, and this continues to happen about once a week. In an interview with Resident 1 on February 2, 2026, at 11:25 AM, the resident stated eating off Styrofoam makes her feel sick, and she does not like Styrofoam. Interview with Employee 1, dietary manager, on February 2, 2026, at 10:15 AM revealed that there was a period beginning December 19, 2025, lasting approximately 2 weeks, where Styrofoam was used at least once a day due to frequent staffing shortages in the department. Employee 1 stated it happens less frequently now.
