F 0684 Provide appropriate treatment and care according to orders, resident’s preferences and goals. Level of Harm - Minimal harm or potential for actual harm Based on review of clinical records, review of facility policies and interviews with staff, it was determined that the facility failed to ensure that medication administration times were following according to physician order for three of three residents reviewed. (Resident R1, Resident R2, Resident R3)Findings include: Review of an undated facility policy, Administration of Medication, under section 6.1 Medication Administration Times, revealed Facility should commence medication administration within sixty (60) minutes before the designated times of administration and should be completed no later than sixty (60) minutes after designated times of administration. Further review revealed Facility should administer medications ordered before meals approximately thirty (30) minutes before mealtimes and should administer medications ordered to be given after meals no later than thirty (30) minutes after a meal has ended. Review of facility meal delivery schedule revealed the following times for all nursing units, Breakfast delivery between 07:35a.m. - 8:25a.m.; Lunch delivery between 11:45a.m. - 12:35p.m.; Dinner Delivery between 4:30p.m. - 6:35p.m. Note: All times are approximate within 10 minutes. Interview with Employee E3, Food Director, on January 22, 2026 at 12:45p.m. revealed no concerns regarding late delivery of food trucks to units, Usually we deliver on time. Interview with Employee E1, Nursing Home Administrator on January 22, 2026 at 1:25p.m. revealed no concerns regarding late delivery of food trucks to units. Confirmed that food trucks are normally delivered on time. No documented evidence of delivery times. Review of Resident R1's clinical record revealed admission date to the facility of November 14, 2025, with diagnosis of Type 2 diabetes. Review of Resident R1's clinical record revealed BIMS (Brief Interview for Mental Statue) score of 15, indicating cognitively intact. Interview with Resident R1 on January 22, 2025 at 10:29 a.m. revealed that medications are late often. Review of Resident R1 MAR (Medication Administration record) dated December 22, 2025 through December 31, 2025 revealed the following: December 22, 2025- Ordered schedule time 5:00p.m.- 7:00p.m; Carvedilol tablet 12.5mg; give 1 tablet by mouth twice a day for hypertension .
