F 0761 Level of Harm - Minimal harm or potential for actual harm Residents Affected - Few Ensure drugs and biologicals used in the facility are labeled in accordance with currently accepted professional principles; and all drugs and biologicals must be stored in locked compartments, separately locked, compartments for controlled drugs. Based on observations, interviews, and record review, the facility failed to store all drugs and biologicals in a locked cart or under the direct observation of authorized staff in an area where residents could access it for one (Wound Care Cart) of five carts reviewed for pharmacy services The facility failed to ensure that LVN A locked the wound care cart after providing wound care on 05/01/2025. This failure could place the residents at risk of accessing/opening the cart causing accidental overdose or misuse of medications. Findings included: Observation on 05/01/2025 at 8:25 AM revealed a cart was parked in the hallway in front of the nurses' station. The cart was not locked and there were some keys on top of the cart. The drawers of the cart were facing the hallway and several staff and residents were passing in front of the cart. All the drawers of the cart could be easily opened and the contents of each drawer could be easily taken. The drawers of the cart contained different types and sizes of dressings, wound cleansers, normal saline, ointments, creams, iodine, collagen, gauze pads, bandages, tongue depressors, tape measures, and disinfecting wipes. During an observation and interview on 05/01/2025 at 8:29 AM, the ADON stated the carts should be locked everytime they were left unattended. She opened one of the drawers of the unlocked cart to check what was inside and what was the cart for. She said the cart was used for wound care. The ADON said the staff should lock the carts before leaving to prevent unauthorized individuals from opening them. She said residents might open them and access or ingest something to which they were allergic. She said, if the carts were left unlocked, any resident, staff, or visitor could open them and get some medications. She said the keys should not be left on top of the carts because even though the carts were locked, others could take the keys and easily open the carts. She said she did not notice the wound care cart was not locked when she came in that morning. She said she would try to find out who left the cart unlocked and the left the keys on top of the wound care cart to educate them of the significance of locking the carts and securing the keys of the carts. She took the keys from the top of the cart and them and placed them in her pockets. In an interview on 05/01/2025 at 9:57 AM, the DON stated any cart should always be locked when left unattended to prevent any residents from opening it and taking something from it. She said the wound care cart had wound cleanser and ointments that could be accidentally drank or ingested that could (continued on next page) Any deficiency statement ending with an asterisk (*) denotes a deficiency which the institution may be excused from correcting providing it is determined that other safeguards provide sufficient protection to the patients. She said if it was the medication cart that was left unlocked, residents could take and ingest some pills, and could result to choking and accidental overdose. She said it was the night nurse that left the wound care cart unlocked and left the keys on top of the cart. She said the expectation was for staff to lock the carts and secure the keys everytime the carts were left unattended. The DON stated she already started an in-service about the importance of locking the cart. Residents Affected - Few In an interview on 05/01/2025 at 10:18 AM, the Administrator stated all the carts should always be locked in protection of the residents. He said the carts should be locked and the keys secured so that no unauthorized individuals could access them. He said the DON already started an in-service pertaining to locking the carts. In an interview on 05/01/2025 at 1:04 PM, LVN A said she did a wound care early morning because a resident was about to go out of the facility for a procedure. She said the wound care coincided with her med pass and some treatments that was why she forgot to make sure that the wound care cart was locked and the keys were secured when she was done with wound care. She said being busy was not an excuse for leaving the cart unlocked and the keys on top of the cart. She said somebody might open the cart and access ointment and creams and accidentally ingest them. She said, next time, she would make sure the carts were locked and the keys were with her before leaving the carts. Record review of the facility's policy, Medication Access and Storage/Drug Destruction Care and Treatment/Pharmacy revised 7/2023 revealed POLICY: It is the policy of this facility to store all drugs and biological in locked compartments . The medication supply is accessible only to licensed nursing personnel, pharmacy personnel, or staff members lawfully authorized to administer medications .
