What the inspector wrote

The following reflects the findings of the California Department of Public Health (CDPH) during the investigation a complaint regarding licensed bed capacity A Class B Citation was written. 22 CCR § 72521 Administrative Policies and Procedures. (a) Written administrative, management and personnel policies shall be established and implemented to govern the administration and management of the facility. 22 CCR § 72607 Patient Capacity (a) A facility shall not have more patients or beds set up for use than the number for which it is licensed except in case of emergency when temporary permission may be granted by the Director or designee. (c) The number of licensed beds shown on a license shall not exceed the number of beds for which the facility meets applicable construction and operational requirements. An announced visit was made into facility on 12/10/2025 to investigate a complaint regarding facility licensed beds capacity. The facility failed to ensure that the number of licensed beds did not exceed the bed capacity the facility was licensed for. This deficient practice had the potential for the facility to admit residents beyond the licensed bed capacity. A review of State of California Department of Public Health indicated that "License... Effective: 4/17/2025. Expires: 4/16/2026. Licensed Capacity: 59. Ina accordance with applicable provisions of the Health and Safety Code of California and its rules regulations, the department of Public Health hereby issues this license to .... To operate and maintain the following Skilled Nursing Facility .... Bed Classification/Services/Stations 59 Skilled Nursing." A review of the facility census dated 12/1/2025, indicated the facilty had 62 beds, 58 residents in house, 1 (one) bed hold, and 4 (four) unoccupied beds (1C, 9B, and 10A, and 18A). A review of the facility census dated 12/1/2025 indicated the following number beds per room. Room - Bed 1 - 1A, 1B, 1C (made/not occupied), 1D 4 - 4A, 4B 6 - 6A, 6B 7 - 7A 8 - 8A, 8B 9 - 9A, 9B (made/not occupied) 10 - 10A (made/not occupied), 10B 11 - 11A, 11B 12 - 12A, 12B 14 - 14A, 14B, 14C, 14D 15 - 15A, 15B 16 - 16A, 16B 18 - 18A (made/not occupied) 19 - 19A, 19B 20 - 20A, 20B 21 - 21 22 - 22A 23 - 23A, 23B 24 - 24A, 24B 25 - 25A, 25B 26 - 26A, 26B 27 - 27A, 27B 28 - 28A, 28B 29 - 29A, 29B 30 - 30A, 30B 31 - 31A, 31B 32 - 32A, 32B 33 - 33A, 33B 34 - 34A, 34B 35 - 35A, 35B 36 - 36A, 36B A review of the facility Client Accommodation Analysis (This form is designed to provide a record of client accommodations approved for licensed care) form completed during 2025 recertification survey, indicated the facilty had 36 residents rooms as follows: Room Number - Approved Capacity - Non-Ambulatory - Ambulatory 1 - 4 - 0 - 3 4 - 2 - 0 - 2 6 - 2 - 0 - 2 7 - 2 - 2 - 0 8 - 2 - 2 - 0 9 - 2 - 1 - 1 10 - 2 - 0 - 2 11 - 2 - 1 - 1 12 - 2 - 1 - 1 14 - 4 - 0 - 4 15 - 2 - 1 - 1 16 - 2 - 1 - 1 18 - 1 - 0 - 0 19 - 2 - 0 - 2 20 - 2 - 0 - 2 21 - 1 - 1 - 0 22 - 1 - 0 - 1 23 - 2 - 1 - 1 Room 1 was listed as 3 beds, whereas Room 1 was observed to have 4 beds Room 9 was listed as 1 bed, whereas Room 9 was observed to have 2 beds Room 10 was listed as 1 bed. Whereas Room 10 was observed to have 2 beds. During a facility actual bed count tour and concurrent record review with the Director of Staff Development (DSD) on 12/10/2025 at 10:44 AM, the facility Client Accommodation Analysis provided by the facility Administrator (ADM) was reviewed. The facility was observed to have 62 beds. During an interview on 12/10/2025 at 1:17PM, ADM stated facility has 62 beds but is licensed for 59 beds. The ADM further stated that the facility maintains a census that does not exceed the licensed Capacity of 59 beds. ADM stated the 3 extra beds are for Isolation during Covid (an infectious [illnesses caused by harmful germs (pathogens) that get into your body] illness caused, characterized by respiratory symptoms (cough, shortness of breath, fever, fatigue, loss of taste/smelling), primarily affecting the lungs. ADM was unable to state how long the facility has had the 3 extra beds. The above violation had a direct relationship to the health, safety, and security of the residents in the facility.
