What the inspector wrote

Title 22 Article 6 Physical Plant, Section 72601(a) 72601. Alterations to Existing Buildings or New Construction. (a) Alterations to existing buildings licensed as skilled nursing facilities or new construction shall be in conformance with Chapter I, Division 17. Part 6. Title 24, California Administrative Code [Reference: 2019 California Building Code Section 1225.2 - New buildings and additions, alterations, or repairs to existing buildings subject to licensure shall comply with applicable provisions of the California Electrical Code, California Mechanical Code, California Plumbing Code and California Fire Code (Parts 3,4, 5 and 9 of Title 24)] and requirements of the State Fire Marshal. On 2/22/2022, the California Department of Public Health (CDPH) made an unannounced visit to the facility to investigate three complaints related to physical environment. The facility failed to obtain the required construction sign-off final approval from the Department of Healthcare Access and Information (HCAI, previously known as the Office of Statewide Health Planning and Development OSHPD; HCAI is the state agency that reviews and approves plans for construction, repairs, renovations, and remodeling made to healthcare facilities to comply with State Building Codes) for the installation / replacement of the kitchen range/oven. On 2/22/2022, at 11:56 a.m., during observation of the kitchen range / oven and concurrent interview, Dietary Cook (DC) stated, "about five years ago the range / oven was replaced." Maintenance Supervisor (MS) present at the time of the interview, stated the HCAI inspector came to inspect the kitchen. MS stated the corporate consultant was in contact with HCAI for the replacement of the range / oven. On 2/22/2022, at 12:50 p.m., an interview was conducted with MS and the Administrator and the HCAI (OSHPD) approval document (Project # S182493-19-00) for the replacement of the range / oven, dated 10/30/2018, was reviewed. The Administrator and MS were not able to locate or provide documentation about the installed/replacement of the range / oven final construction sign-off approval from HCAI. On 2/22/2022, at 1:22 p.m., during an interview, the Administrator stated, "I was aware the kitchen range was replaced and HCAI was involved in the project." The Administrator stated, once the project is completed, our service center will send us the final paperwork. We did not receive the final HCAI approval document. The facility failed to obtain the required construction sign-off final approval from the HCAI for the installation / replacement of the kitchen range/oven. The above violation had a direct relationship to the health, safety, and security of all residents.
