What the inspector wrote

California Code, Health and Safety Code - HSC § 1422.6 Each skilled nursing facility and intermediate care facility shall post a copy of the notice required pursuant to Section 9718 of the Welfare and Institutions Code in a conspicuous location in at least four areas of the facility, as follows: (a) One location that is accessible to members of the public. (b) One location that is used for employee breaks. (c) One location that is next to a telephone designated for resident use. (d) One location that is used for communal functions for residents, such as for dining or resident council meetings and activities. The facility failed to post the State Long-Term Care Ombudsman Program (residents' advocacy group) notice in the residents' dining room and the employees' lounge. As a result, the residents and staff were placed at risk of not knowing how to reach the Ombudsman Program to advocate for the residents' rights. On 4/6/2021 at 10 a.m., during a Resident Council Meeting, three residents stated they were not aware of where the Long-Term Care Ombudsman contact information was. On 4/6/2021 at 12:03 p.m., during a concurrent observation and interview, the Director of Nursing (DON) confirmed there were no posted Ombudsman signs in the employees' lounge and in the residents' dining room. The DON stated there should be one posting in each location. On 4/8/2021 at 2:58 p.m., during a concurrent observation and interview, the Administrator confirmed there was no Ombudsman posting in the employees' lounge and the posting in the residents' dining room was not put back. The facility failed to post the State Long-Term Care Ombudsman Program (residents' advocacy group) notice in the residents' dining room and the employees' lounge. As a result, the residents and staff were placed at risk of not knowing how to reach the Ombudsman Program to advocate for the residents' rights. The above violation had a direct or immediate relationship to the health, safety or security of all residents in the facility.
