What the inspector wrote

On 04/23/2025 at 12:15 PM, Licensing Program Analyst (LPA) David Doidge arrived unannounced to gather documentation on residents currently in the facility. LPA met with Caregiver Vitelio Orellana and explained the purpose of the visit. Administrator Ferdinand Gutierrez had stepped out. Caregiver informed LPA there is only 6 residents in the facility. LPA obtained copies of the Identification and Emergency Information sheets for all 6 residents. LPA called Licensee/Administrator Victoria Puruganan to inform her of the visit. LPA also asked Licensee to have the Administrator email an LIC500 to the LPA as current staff did not have it available. No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
