What the inspector wrote

On 7/20/2021 at 03:30 PM, Licensing Program Analysts (LPAs), L. Hall and G. Luk arrived unannounced to conduct an initial 10-day complaint investigation and to deliver complaint findings for the above allegations. LPAs met with Executive Director, Deborah Savoie, and explained the reason for the visit. During the course of investigation, LPAs interviewed staff, resident and witness. LPAs obtained and reviewed residents' roster, and resident's file including physician's reports, care plans, emergency information. Interviews with witness, staff, and R1 indicated that involved party was an employee of an outside agency. This agency has investigated the complaint on the above allegations, we have found that the complaint was UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without reasonable basis. We have therefore dismissed the complaint. Exit interview conducted and a copy of this report provided.
