Based on observation, the licensee did not comply with the section cited above. LPAs observed various items in the front yard of the facility: broken dreeser, land mower, garbage, etc. which poses a potential health, safety or personal rights risk to persons in care.

87464 Basic Services (f) Basic services shall at a minimum include: (6) Arrangements to meet health needs, including arranging transportation, as specified in Section 87465, Incidental Medical and Dental Care Services.-This requirement has not been met as evidenced by: Based on LPAs records review and interviews, Licensee/Administrator failed to ensure R2’s vaccinations were current and provide proof of residents dental and ophthalmology exams.

87411 Personnel Requirements – General(a)Facility personnel shall at all times be sufficient in numbers...Additional staff shall be employed... to perform office work, cooking, house cleaning, laundering...-This requirement is not met as evidenced by Based on interviews and review of records, the Administrator/Licensee did not comply with the above Regulation by facility personnel not being sufficient in numbers.

Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. Based on record review, Licensee failed to organize and clean the facility which poses potential health and safety risk to clients in care.

80087(g): Buildings and Grounds: Disinfectants, cleaning solutions, poisons, firearms and other items that could pose a danger if readily available to clients shall be stored where inaccessible to clients.This requirement was not met as evidence by: Based on LPAs observation, Administrator failed to ensure cleaning products are was inaccessible to clients, which poses an immediate health and safety risk to clients in care.

80087 Buildings and Grounds(a) The facility shall be clean, safe, sanitary and in good repair at all times for the safety and well-being of clients, employees and visitors.This requirement is not met as evidence by: Based on LPAs observation, Administrator failed to ensure that kitchen cabinet, faucet sink and second floor screen door by the patio is in good repaid, which poses an potentia health and safety risk to clients in care.
