Pre-licensing visit·May 8, 2025

On 05/08/2025 at 1:20PM, Licensing Program Analyst (LPA) T. Syess-Gibson arrived announced to conduct a Pre-licensing Inspection. LPA met with Licensee, Robert Johnson and Administrator, Filicia Johnson. LPA explained the purpose of the visit. LPA toured facility including but not limited to resident's bedrooms, bathrooms, living room, dining area, kitchen, garage, and outdoor area. LPA observed lighting in all rooms. LPA observed facility had some perishable and non-perishable food supply. Licensee will purchase additional food supplies once facility is licensed. Smoke and Carbon monoxide detector was observed in operating condition. First aid kit was complete. Fire extinguisher was observed to be full and was purchased on 05/23/2024. Hot water was measured at 109.3 degrees F. Emergency disaster plan was complete. The following will need to be completed before recommending licensure to Centralized Application Bureau (CAB): 1.)LPA observed facility does not have mattress pads for all resident beds. 2.)LPA observed facility has washing machine/dryer in the garage obstructing passageway. Continue on LIC809C Continued from LIC809 3.)LPA observed facility does not have an exit sign in the backyard identifying emergency exit. 4.)LPA observed facility does not have a properly locked cabinet for sharps. 5.)LPA observed facility does not have a signal system. 6.) LPA observed facility does not have a dementia plan to address behaviors such as ingestion of toxic chemicals and wandering behaviors. Licensee/Applicant will submit proof of corrections to CCLD on or before 05/15/2025. LPA will conduct COMP III during next visit. Exit interview conducted and a copy of this report provided.
