Based on observation, interview, record review, the licensee did not comply with the section cited above in 2 out of 2 medications: Medicatons: Mirtazapine 7.5 MG tablet Take 1 tablet by mouth once per day and Melatonin 3 MG Take 1 tablet by mouth at bedtime was not initialed once successfully administered in Resident MAR which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above in 5 out of 5 resident bathroom shower areas did not contain non-slip mats or non-slip strips which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above in 3 out of 5 residents did not contain the Safeguard for Property Values document in resident files, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in resident files reviewed showing residents with health conditions do not have the required exceptions/care plans in place. This poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that 5 of 5 files reviewed did not have appraisals as required. This poses a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidence by: LPA observation of Incident Reports, SOC 341 reports, and resident files. The licensee did not comply with the section cited above in that the facility did not provide a SOC 341 to CCL for incident occurring on 10/26/2024. Facility did not report an incident occurring on 8/31/24 in the required time frame. This poses a potential health, safety and or personal rights risk to persons in care.

87470 Infection Control Requirements(a) A licensee shall ensure that infection control practices are maintained as follows:… This requirement was not met as evidence by LPA interview of Administrator stating there was an outbreak of COVID in the facility and due to staff shortages they were not able to follow guideline set forth by CCL, CDC and/or CDPH. Observation of training records indicated that 3 of 4 new staff have not had infection control practices training. This poses a potential health, safety and or personal rights risk to residents in care.
