What the inspector wrote

On 3/6/25 Licensing Program Analyst (LPA) M. Garza completed an unannounced case management visit. LPA met with Administrator, Jenny Penner, explained reason for visit and was permitted entry into the facility. A health and safety was completed on residents in care. Residents observed in common areas and in rooms. This case management is being conducted as a follow up visit to a case management completed 2/27/25 for R1. This visit is for additional information on R1, documents was requested (physicians report, incident report, hospital discharge records, therapy notes, R1 care plan, medication list, face sheet for R1) and interviews were conducted (Administrator, S1 and S2) and follow up will be needed. At this time this case management needs further details. No deficiencies cited during todays visit. Exit interview completed with Administrator, Jenny Penner. A copy of this report provided.
