Exit door leading to the garage does not open completely and blocked with items. Small chest freezer in garage observed to be plugged into a extension cord. Both side yard gates observed with sliding locks and missing pull cord not allowing accessibility. This poses an immediate health, safety or personal rights risk to persons in care.

Based observation, the licensee did not comply with the section cited above in that chemicals/supplements observed under kitchen sink and in pantry unlocked and accessible to residents in care. This poses a potential health, safety or personal rights risk to persons in care.

Based on interview and observation, the facility failed to notify the department the facility bathroom is being repaired which poses a potential health and safety risk for the residents in care.

Based on interviews, observation, and records reviewed, R2’s medications Cholecalciferol and Rosuvastatin were administered daily and were not recorded in the resident’s MARs which poses a potential health and safety risk for the person in care.

Based on observation, the licensee did not comply with the section cited above when LPA and Administrator Assistant observed R3’s medication that required refrigerated stored unlock in the refrigerator accessible to residents poses an immediate health, safety or personal rights risk to person in care.

Based on observation, LPA and Licensee observed S1’s medication stored the refrigerator unlock which poses an potential health and safety risk to the residents.

Based on observation and records reviewed, R1 was observed in a full rail bed with no physician’s order which poses a potential health and safety risk for the person in care.

Based on LPA observation, the licensee did not comply with the section cited above in that the portable fireplace gets hot to the touch and is not surrounded by a screen, which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA observation, the licensee did not comply with the section cited above in, which garage was observed with clutter, 2 of 2 side gates do not open from the inside and are not self latching. This poses a potential health, safety or personal rights risk to persons in care.

87303 Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement was not met as evidence by: LPA observation and interivew conducted with Administrator. LPA observed three large boxes containing clothes and personal hygeine products next to the heating/cooling unit in the hallway closet.

At 1:28 p.m., LPA and Assistant Administrator observed side exit fence nonfunctioning. Side gate exit not able to open without falling. Side gate exit was observed not self-closing and self-latching. This poses a potential health, safety, and personal rights risk to residents in care.
