What the inspector wrote

Based on interviews conducted, the allegations: resident developed pressure injuries while in care, staff did not assist resident with ambulating, facility did not meet resident's dietary needs are UNSUBSTANTIATED. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. No deficiencies issued. An exit interview was conducted. A copy of this report was discussed and provided to Administrator, Sarah Dennis, whose signature on this form confirms receipt of this document.

This requirement was not met as evidenced by: Based on record review, the facility did not ensure the requirements for section 87465(a)(4) were met when facility staff administered medications belonging to R1 to R2, which poses an immediate health and safety risk to residents in care.
