The facility was not aware R1 went AWOL until approximately at 10:15 PM Fresno PD notified the facility. This is poses an immediate health and safety risk to persons in care.

This was not observed as evidenced by: Facility failed to submit incident report in timely manner to Licensing, which posses potential safety risk to residents in care.

This requirement was not met as evidenced by: Based on record review, the facility did not ensure the requirements for section 87465(a)(4) were met when facility staff administered medications belonging to R1 to R2, which poses an immediate health and safety risk to residents in care.

  • Keep prescriptions in original containers

    Based on observation and interviews, the licensee did not comply with the section cited above in when medications for residents were removed from the original packaging and placed in envelopes due to the bottles not fitting in medication cart which poses a potential health, safety or personal rights risk to persons in care.

  • Assist residents with self-administered medication

This requirement was not met as evidenced by: Based on interviews and record reviews, the licensee did not ensure the requirements for the above regulation were met when S1 administered medications that were prescribed to R1 to R2. This poses an immediate health and safety risk to persons in care.
