On this date, Licensing Program Analyst (LPA) D. Ayers arrived at the facility unannounced to deliver complaint findings. LPA identified himself and discussed the purpose of the visit with Administrator Rose Riemer. During the course of the investigation, the Department conducted interviews with staff, interviews with residents, and reviewed records. Residents stated that their needs are being met and they are satisfied with the care they receive at the facility. Facility staffing is appropriate to meet the residents' care and supervision needs. Facility staff stated they are able to meet the needs of the residents. Resident files were updated. The allegations are Unsubstantiated. No deficiency was observed. Exit interview conducted, and a copy of this report provided via email.
