Based on record review, the licensee did not comply with the section cited above in that record review of R1’s file did not have a completed medical assessment within the last year in their file. This poses a potential health safety and or personal rights risk to residents in care.

Certified administrator requirements and substitute coverage

Based on record review, the licensee did not comply with the section cited above in that records reviewed showing Administrator on file does not have a current administrator certificate. This poses an immediate health safety and or personal rights risk to residents in care.

Request a transfer of criminal record clearance

Based on records reviewed, the licensee did not comply with the section cited above in that S2 was fingerprint cleared but not associated to the facility. S2 worked with residents in care on 12/23/25. This poses an immediate health, safety or personal rights risk to persons in care. which poses an immediate health, safety or personal rights risk to persons in care.

Applicant and licensee fees are charged by statute

Based on record review, the licensee did not comply with the section cited above in that the facility has a past due amount of $3463 in licensing fees. This poses a potential health, safety or personal rights risk to persons in care.

Diet quality and preparation in safe healthful manner

87555 General Food Service Requirements (a) The total daily diet shall be of the quality and in the quantity necessary to meet the needs of the residents and shall meet the Recommended Dietary Allowances of the Food and Nutrition Board of the National Research Council. All food shall be selected, stored, prepared and served in a safe and healthful manner. This requirement was not met as evidence by: LPA observation. The licensee did not comply with the section cited above in that the LPA did not observe a sufficient amount of fresh fruits and vegetables or protein in the refrigerator/pantry for the 6 resdents in care.

(b) A licensee shall ensure the following requirements are met in providing any internet access device for resident use: (1) The device shall be available in a manner that allows a resident to access it for discussion of personal or confidential information with a reasonable level of personal privacy. This requirement is not met as evidenced by:Based on LPA observation, the licensee did not comply with the section cited above in that the facility has a computer in the living room that does not have a camera and is inaccessible to residents. Facility does not have a device for resident use. This poses an immediate health, safety or personal rights risk to persons in care.

87555 General Food Service Requirements(b) The following food service requirements shall apply:(3) Between-meal nourishment or snacks shall be made available for all residents unless limited by dietary restrictions prescribed by a physician. This requirement was not met as evidence by: LPA observation of limited snacks (3 pks of cookies) only being available to residents in care.

Maintain facility in clean, safe, sanitary condition

(a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement was not met as evidence by: Dishwasher non-functioning. Pool non-functioning & has dirty water. Mattress in R1's bedroom in need of replacement. R1's bedroom in need of touch up paint (near bed & near closet). Bedroom #2 restroom has broken tiles. Laundry room observed with holes in wall. Vent in laundry room in need of cleaning. Bedroom #3 door in need of repair. Bathroom #2 observed with bathtub and sinks in need of resurfacing. Bathroom #2 cabinets in need of repair or replacement. Bathroom #2 toilet missing toilet cover. Bathroom #3 in need of paint around toilet. Backyard patio roof observed with holes in need of repair. Garage observed with debris in need of removal. Large left side gate broken and in need of repair. Wires exposed on HVAC unit in need of repair. Right side gate does not latch properly and observed with broken board in need of repair. Driveway observed with missing concrete in need of repair. Kitchen observed with spider webs/dust above stove. Kitchen tiles broken and in need of replacement near sink and on counter. These poses a potential health, safety and or personal rights risk to residents in care.

87217 Safeguards for Resident Cash, Personal Property, and Valuables(h) Immediately upon admission, residents' cash resources entrusted to the licensee and not kept in the licensed facility shall be deposited in any type of bank, savings and loan or credit union account, which is maintained separate from the personal or business accounts of the licensee, provided that the account title clearly notes that it is residents' money and the resident has access to the money upon demand to the licensee. This requirement was not met as evidence by: Based on LPA observation, the licensee did not comply with the section cited above in that residents do not have personal accounts and money is being comingled. Facility staff is not providing P&I and clothing allowance money to residents in a timely manner. This poses a potential health, safety and or personal rights risk to persons in care.

This poses a potential health, safety or personal rights risk to persons in care.

Based on LPA observation, the licensee did not comply with the section cited above in that staff files reviewed did not have required CPR/1st Aid certificates. This poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA interviews, the licensee did not comply with the section cited above in that facility does not have activities for the residents. This poses a potential health, safety or personal rights risk to persons in care.

Dishwasher non-functioning. Pool non-functioning & has dirty water. Mattress in R1's bedroom in need of replacement. R1's bedroom in need of touch up paint (near bed & near closet). R1's bedroom missing nightstand. Bedroom #2 restroom has broken tiles and shower in need of cleaning. Laundry room observed with holes in wall. Vent in laundry room in need of cleaning. Bedroom #3 door in need of repair. Bathroom #2 observed with bathtub and sinks in need of resurfacing and shower in need of cleaning. Bathroom #2 cabinets in need of repair or replacement. Bathroom #2 toilet missing toilet cover. Bathroom #3 in need of paint around toilet. Bedroom #4 missing nightstand. Backyard patio roof observed with holes in need of repair. Freezer in garage in need of defrosting. Box spring in garage observed with hole and in need of disposal. Oxygen tank without stand/strapped down observed in garage in need of removal/disposal. Garage observed with debris in need of removal. Back door window observed broken in need of repair. Large left side gate broken and in need of repair. Wires exposed on HVAC unit in need of repair. Right side gate does not open/close properly in need of repair. Tiles in front walkway missing or cracked in need of repair/replacement. Driveway observed with missing concrete in need of repair. Back door trim in need of replacement. Kitchen observed with spider webs/dust above stove and on fan above table. Bathroom #3 observed with smell of urine. Kitchen tiles broken and in need of replacement near sink. . This poses a potential health, safety or personal rights risk to persons in care.

Based on LPA observation of staff files not having required training, the licensee did not comply with the section cited above in 2 of 2 staff files reviewed. This poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA observation of the emergency disaster plan, the licensee did not comply with the section cited above in that the plan was incomplete and without the required documentation. This poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA observation, the licensee did not comply with the section cited above in that the facility has a computer in the living room that does not have a camera and is inaccessible to residents. Facility does not have a device for resident use. This poses an immediate health, safety or personal rights risk to persons in care.

87303 Maintenance and Operation (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement was not met as evidence by: LPA observation of the following; Light switches in facility non-functioning. Clutter observed along fence (boxes, bed frame, boards and window frame, recliner chair). Side fences do not properly open/close and/or lock/latch. Emptied pool has dirty water in deep end. Garage observed with clutter and debris.

Provide adequate lighting for room safety and comfort

87303 Maintenance and Operation (d) There shall be lamps or light appropriate for the use of each room and sufficient to ensure the comfort and safety of all persons in the facility. This requirement was not met as evidence by: LPA observation of the following; Light switches in facility non-functioning, Light bulbs in room #1 missing from light sockets. Room #4 does not have sufficient lighting and making noise. Light bulbs need replacement in room and in restroom.

Based on interview andrecord review, the licensee did not comply with the section cited above for three out of six residents, which poses a potential health, safety or personal rights risk to persons in care.

Ensure hazardous items are locked and not unattended

Based on observation, the licensee did not comply with the section cited above in one out of two bathrooms and the laundry room, which posed a potential health, safety or personal rights risk to persons in care.

Encourage resident participation in varied planned activities

Based on observation and interview, the licensee did not comply with the section cited above in for six out of six residents, which poses a potential health, safety or personal rights risk to persons in care.
