Licensing Program Analyst (LPA) Sandra Urena conducted a subsequent unannounced, Case Management-Other visit to the unlicensed facility. The LPA was allowed entry by staff. The LPA spoke with staff and explained the reason for the visit. Staff called the operator on the phone, and the LPA informed the Operator Ruzanna Armine of the reason for the visit. The purpose of this visit is to conduct the monthly follow-up well check on the status of one Tenant who was observed to be in need of care and supervision, and to ensure no additional are tenants residing at the unlicensed facility. On 01/02/2025, Licensing Program Analysts (LPAs) Sandra Urena, Erica Mosley, and Adult Protective Services (APS), Social Worker (SW), Adrian Garcia, conducted an unannounced visit at the above location at 8:30 a.m. The Community Care Licensing Division (CCLD) received a complaint regarding unlicensed care being provided at this address . On 01/24/2025, the Operator submitted an application to the Centralized Application Bureau (CAB). The LPA conducted a physical plant tour of the facility and spoke with Tenants. Tenants reported they were doing ok. At the time of the visit, T1 was still residing at the unlicensed facility. No additional tenants were observed at the time of the visit. The Operator was not available during the visit. The staff signed the report. No citation were issued. Exit interview was conducted and a copy of the report was issued.
