What the inspector wrote

Licensing Program Analyst (LPA) Sandra Urena conducted a subsequent unannounced, Case Management-Other visit to the unlicensed facility. At the time of the visit, the LPA found the driveway gate locked. The LPA was allowed entry by staff. The LPA spoke with staff and explained the reason for the visit. Staff called the Applicant on the phone, and the LPA informed the Applicant Meri Tarposhyan of the reason for the visit. The purpose of this visit is to conduct the monthly follow-up well check on the status of one T1 who was observed to be in need of care and supervision, and to ensure no additional are tenants residing at the unlicensed facility. The Community Care Licensing Division (CCLD) received a complaint regarding unlicensed care being provided at this address, and the complaint was substantiated on 01/02/2025. On 01/24/2025, the Operator submitted an application to the Centralized Application Bureau (CAB). Per th eApplicant , the LAFD Inspector conducted a second visit for the corrections found during the first visit. The LPA conducted a physical plant tour of the facility and checked on T1, and T1 expressed doing well. No additional tenants were observed at the time of the visit. The LPA reminded the Applicant not to take in tenants who are in need of care and supervision in which a license is required. The LPA reminded the Applicant that operating an unlicensed location is in violation of section 1569.10 and/or 1569.44 of the Health & Safety Code.During today’s visit, the LPA discussed with the Applicant the pending documents as they are described in a letter sent by CAB to the Operator on September 11, 2025. The pending documents are: 1. Approved Fire Clearance: Due in 30 days from date of notice (09/11/2025). 2.Administrative Organization (LIC309). 3.Financial Information Release and Verification (LIC 404), and (LIC 401, line 37). The Applicant was not available during the visit. The Applicant instructed staff to sign the report. Exit interview was conducted and a copy of the report was issued.
